الجسم بيشتغل
عشان “يأجل” المشكلة
مش يحلّها

• BP “normal” بسبب vasoconstriction
مش perfusion حقيقي

• Tachycardia
مش stability… ده stress response

• SpO₂ كويس
بس oxygen delivery questionable

• Subtle confusion أو agitation
قبل أي drop واضح

• Rising lactate…
while everything looks “fine”

It doesn’t warn you

⚠️ Clinical Insight (Integrated)

المريض اللي بدأ يتلخبط أو يتـ agitate
ده مش “behavior issue”

ده ممكن يكون:
👉 Hypoxia
👉 Hypoperfusion
👉 بداية collapse

🚨 وهنا الغلطة بتحصل:

نمسك الـ agitation بـ restraints أو sedation…
وننسى نسأل “ليه؟”

Restraints ممكن تحمي المريض momentarily
لكن…

❌ مش بتعالج السبب
❌ وممكن تخبي deterioration

Treat the cause… not just the movement

Don’t ask:
“Is he stable?”

Ask:
👉 What is he compensating for?
👉 Why is he getting agitated?
👉 What happens when this fails?

Stable monitor…
Normal vitals…
Everything looks fine.

But in ICU…
the danger doesn’t always scream.

Sometimes… it whispers.

📉 Rising RR
📉 Subtle agitation
📉 Dropping urine output
📉 Increasing support with “normal” BP

And the biggest trap?
“Vitals are okay… keep observing.”

💣 The problem isn’t the numbers…
It’s believing them alone.

🧠 ICU Mindset:
• Monitor reassures
• Trend warns
• Patient tells the truth

⚠️ Remember:
Normal ≠ Safe
Stable ≠ Not Sick

🚨 Early deterioration is silent…
Until it’s too late.

الـ monitor بيقولك “دلوقتي”
بس مش بيقولك
المريض رايح على فين

📉 Clinical Red Flags (Easy to Miss)

• RR بيزيد تدريجي
حتى لو SpO₂ لسه كويس

• Increased work of breathing
بس saturation “مطمّنة”

• Subtle agitation أو ↓ level of consciousness

• BP طبيعي…
بس محتاج support أكتر كل شوية

• Urine output بيقل
من غير ما حد ياخد باله

• Rising lactate…
before the crash becomes visible
