💡 Laryngectomy 101: What is an HME?

An HME is placed directly over your stoma, and as air is exhaled, heat and moisture from your breath are captured in the HME. The heat and moisture are added back to the air you inhale before it passes to your lungs. HMEs add slight resistance to breathing to help your lungs function well.

HMEs also help keep large particles out of your lungs, such as insects or food crumbs. It is important to use an HME 24 hours a day, 7 days a week. Most people who consistently use a Provox Life HME 24/7 experience fewer problems with coughing and mucus production.

How does a HME work?

An HME contains a foam that has been treated with a special compound called calcium chloride. This traps heat and moisture from the air you breathe.

When you breathe out through your stoma, the foam collects and saves the humidity and warm air.

When you breathe in, the humidity and warm air that are saved in the foam are given back to the air you breathe in.

In other words, an HME ‘conditions’ the air you breathe keeping it at the right temperature and humidity. This helps to support your lungs to function well. Never wash or rinse out your HME as it will render the HME ineffective.

Many people find that wearing an HME all the time can help them live a better life and go back to doing many of the things they did before their surgery. This is because an HME helps them produce less mucus, cough less, feel less irritation in their windpipe, and breathe more easily.

There are different types of HMEs specially designed to work better in different situations.

💡 𝗟𝗮𝗿𝘆𝗻𝗴𝗲𝗰𝘁𝗼𝗺𝘆 𝟭𝟬𝟭: 𝗛𝗼𝘄 𝗱𝗼 𝘆𝗼𝘂 𝗰𝗮𝗿𝗲 𝗳𝗼𝗿 𝘆𝗼𝘂𝗿 𝘀𝘁𝗼𝗺𝗮?

Peristomal skincare is vital for keeping the skin around your stoma healthy. Factors like dry air, mucus and adhesives can all cause irritation. Specifically, the incorrect daily application and removal of adhesives can damage the skin and lead to discomfort and problems with the adhesive sticking to the skin. The umbrella term for adhesive related skin injuries is Medical Adhesive Related Skin Injuries, or MARSI.

𝗪𝗵𝗮𝘁 𝗶𝘀 𝗮𝗻 𝗮𝗱𝗵𝗲𝘀𝗶𝘃𝗲?
An adhesive or baseplate is the flexible plastic patch with a sticky backing that is attached around your stoma. The adhesive provides a stable surface and mounting point to attach Heat and Moisture Exchangers (HMEs) which are used to heat and humidify inhaled air. Adhesives are available in different sizes, shapes and with different types of adhesive formulations depending on your individual needs.

𝗪𝗵𝗮𝘁 𝗰𝗮𝘂𝘀𝗲𝘀 𝗶𝗿𝗿𝗶𝘁𝗮𝘁𝗶𝗼𝗻?
Irritation can be caused by a variety of different factors. With adhesives in particular, the removal of the adhesive also removes a small amount of skin cells, even in those with healthy skin.
When the adhesive to skin bond is stronger than the bond between skin layers, the adhesive can pull the top layers of skin (epidermis) from each other or pull the epidermis away from the layers of skin underneath (dermis).
Irritation can also be caused by leaving the adhesive on for too long and not allowing the skin to breathe. Mucus or other secretions from the stoma can also find their way in the small spaces between an incorrectly applied adhesive and the skin.

👃 Your nose does more than just smell – it heats, humidifies, and filters the air you breathe. In this way, you can be sure the air is at the right temperature and contains enough moisture when it reaches your lungs for them to function properly.

🌬️ After your total laryngectomy, you will breathe through the stoma in your neck so these nasal functions are lost. Breathing through an open stoma causes the temperature and humidity in your lungs to drop. The lungs react to this by producing more mucus, meaning you have to cough more (similar to having a cold) and your windpipe can feel irritated.

🩺 Heat and Moisture Exchangers (HMEs) have been developed to compensate for the functions of your nose. They help to rebalance the “climate” in your lungs.

💡 Laryngectomy 101: How can you speak after a laryngectomy?

We rely on our voices to express our thoughts and feelings. A total laryngectomy means your larynx is removed – including your vocal cords. Right after a laryngectomy, the quickest way to communicate with family, friends and healthcare professionals will be through the use of writing and gestures. It may help to practice some simple gestures and ways of communicating together with friends and family prior to your surgery. There are also apps available for smartphones that can help translate text to speech.

Losing your natural voice can initially be quite upsetting, and will have an impact on both your ability to communicate and your sense of identity. But the good news is that there are several ways to regain your voice and ability to speak after a laryngectomy. In most cases you will already have been in contact with your speech and language therapist prior to your surgery, he or she will give you advise on voicing methods that can be learned after surgery to develop your new voice.

There are 3 different methods for voice rehabilitation following a laryngectomy:
