Why Do Some Men Need a Biopsy During a Cystoscopy?

Imagine hearing these words during your appointment: "We may need to take a small sample." For many men, that's when anxiety suddenly increases.

Sometimes a cystoscopy shows an area that needs a closer look. In these cases, the doctor may take a biopsy.

👉 Biopsy = a small sample of tissue removed for examination under a microscope.

Why is a biopsy done? Doctors may want to investigate:
• unusual growths
• abnormal-looking tissue
• persistent inflammation
• unexplained changes inside the bladder

What happens next?
The sample is sent to a laboratory where specialists examine the cells.

This helps doctors better understand what is causing the abnormal appearance.

Important to know:
Needing a biopsy does not automatically mean cancer is present. A biopsy is a tool used to find answers.

Why Do Some Men Need Sedation for a Cystoscopy?

One of the most common questions men ask is: "Can they put me to sleep for this?"

The answer depends on the situation.

Not all cystoscopies are performed the same way.

Some are done while the patient is awake using numbing jelly.

Others may involve sedation or anaesthesia.

👉 Sedation = medication that helps a person relax or become sleepy during a procedure.

Doctors may consider sedation when:
• A more complex procedure is planned
• Additional treatments are being performed
• The patient is extremely anxious
• Other medical factors are involved

Why are many cystoscopies done awake?
The procedure is often quick and can be completed safely without putting the patient to sleep.

Why Do Doctors Use Water During a Cystoscopy?

Many men are surprised to learn that water is often used during a cystoscopy.

So, what is the purpose?

During a cystoscopy, doctors need a clear view of the inside of the bladder.

To do this, sterile fluid is gently introduced into the bladder.

👉 Sterile fluid = a specially prepared fluid that is free from germs.

Why is it needed?
The bladder is normally a collapsed organ when empty. The fluid helps:

• expand the bladder slightly
• smooth out folds in the bladder wall
• improve visibility
• help doctors spot abnormalities

Why some men feel uncomfortable: As the bladder fills, many men experience:
• pressure in the lower abdomen
• a strong urge to urinate
• a feeling of fullness

Important to know:
This sensation is usually expected and helps the doctor perform a thorough examination. The fluid is not there to cause discomfort. It helps doctors see the inside of the bladder more clearly and accurately.

Why Do Some Men Say They Felt Fine Until They Got Home After a Cystoscopy?

Many men leave the clinic thinking: "That wasn't nearly as bad as I expected."

Then, a few hours later, things feel very different. Why?

During and immediately after the procedure, many people are focused on getting through the test.

Once they return home and begin normal activities, they may become more aware of symptoms. Common experiences include:

• burning when urinating
• increased urgency
• mild soreness
• discomfort at the tip of the p***s

Why can this happen?
The urethra may become temporarily irritated during the procedure.

👉 Irritation = temporary sensitivity or inflammation of tissue.

As urine passes through the area afterwardafterwards, symptoms may become more noticeable.

Another reason:
Many men are so focused on the procedure itself that they pay less attention to minor discomfort until later.

N: B Experiences vary from person to person. Some men have very few symptoms afterwards, while others notice temporary discomfort for a short period.

Feeling fine immediately after the procedure does not always mean you won't notice symptoms later.

Can Anxiety Make a Cystoscopy Feel More Painful?

Two men undergo the same cystoscopy.

One says, "It was uncomfortable but manageable."

The other says: "It was much worse than I expected."

Why can experiences be so different?


Anxiety can affect how we experience medical procedures. When we become nervous, the body responds automatically.

What happens when anxiety increases? The body may:
• tighten muscles
• increase alertness to sensations
• heighten awareness of discomfort

During cystoscopy, tension in the pelvic area can sometimes make the procedure feel more uncomfortable.
👉 Pelvic muscles = muscles that support the bladder and urinary system.

Why fear matters:
Many men arrive for the procedure after reading frightening stories online or hearing difficult experiences from others. This can make them expect severe pain before the test even begins.

Important to know: Feeling anxious does not mean the pain is "all in your head."
The experience is real. But anxiety can influence how strongly discomfort is perceived.
