Safe Surgical Planning Before Pregnancy

In our patient who presented with infertility, a 7 cm endometrioma (chocolate cyst) was detected in the right o***y, along with fluid accumulation in the left fallopian tube (hydrosalpinx). Since both conditions can directly contribute to infertility, a surgical decision was made.

The patient had previously undergone two open abdominal surgeries, which significantly increased the risk of intra-abdominal adhesions. For this reason, the surgical approach was planned as open surgery.

Our goal was clear:
to prepare the patient for pregnancy without increasing adhesion risk and without causing unnecessary tissue damage.

The key principles are simple but critical:
the right patient, the right timing, and the right surgical approach.
The outcome is shaped accordingly.

Surgical Approach in Ovarian Cyst Operations

In ovarian cyst surgeries, our first choice is always laparoscopic surgery. However, each patient must be evaluated individually.
This patient had previously undergone two surgeries with a lower midline abdominal incision. Such procedures significantly increase the risk of intestinal adhesions, which may make laparoscopic surgery unsafe.

For this reason, we opted for open surgery.
During the operation, we removed the right ovarian cyst along with the damaged fallopian tube. With this approach, we were able to prepare our patient for a safer and healthier pregnancy potential.

In summary, the choice of surgical technique should be based on previous surgical history and existing risks.
There is no one-size-fits-all approach. Proper planning is the key to successful outcomes.

Excess Weight and IVF Success

Excess body weight negatively affects the success of IVF treatment.
This patient applied to our clinic at the age of 44 with significant overweight.

Our first step was weight management. We supported the patient in losing weight over a 3-month period, and only after achieving this did we proceed with IVF treatment.

During the procedure, 10 oocytes were retrieved.
The resulting embryos will be sent for genetic testing, and embryo transfer will be planned based on the results to maximize success and safety.

Abnormal Uterine Bleeding Caused by Endometrial Polyps

Endometrial polyps are one of the most common causes of increased menstrual bleeding.
In this patient, there had been a complaint of excessively heavy menstrual bleeding for some time.

During the gynecological examination, a 17 mm polypoid lesion was detected inside the uterus.
The treatment plan is hysteroscopy, a minimally invasive method that allows us to directly visualize the uterine cavity and completely remove the polyp in a precise and controlled way.

Hysteroscopic polyp removal is both effective and safe, and it helps relieve symptoms while protecting the uterus.
