Left Atrial Enlargement

الصورتان دول لنفس الـ chest radiograph وبيوضحوا وجود Left atrial enlargement secondary to mitral valve disease.

في الحالة الطبيعية الـ left atrial appendage بيكون concave، لكن مع التضخم بيحصل:
bulging واضح للخارج بدل الـ normal concavity.

في الأشعة هنلاحظ:
• Bulging of the left atrial appendage (محدد باللون الأحمر في الصورة اليمنى)
• Right heart border يبدو displaced more to the right من الطبيعي بسبب تضخم الـ left atrium
• ممكن كمان يصاحب الحالة signs أخرى زي widening of the carinal angle (في بعض الحالات)

أهم سبب:
Mitral valve disease (خصوصًا mitral stenosis)

Diagnosis: Left Atrial Enlargement

Remember:
Bulging left atrial appendage + mitral valve disease = Left atrial enlargement

Cardiomegaly

الصورتان دول عبارة عن PA chest radiographs وبيوضحوا وجود Cardiomegaly.

في التقييم بنستخدم الـ cardiothoracic ratio (CTR)، وهو نسبة عرض القلب إلى عرض الصدر.

في الأشعة هنلاحظ:
• الخطوط المتقطعة السوداء بتحدد حدود القلب
• السهم الأحمر القصير يمثل عرض القلب
• السهم الأسود الطويل يمثل عرض الصدر بالكامل
• عرض القلب أكبر من نصف عرض الصدر (>50%) → إذًا يوجد cardiomegaly

كمان ممكن نلاحظ:
• وجود Median sternotomy wires → دلالة على جراحة قلب سابقة

مهم جدًا:
لازم تكون الصورة PA view لأن AP view ممكن يعمل overestimation لحجم القلب.

Diagnosis: Cardiomegaly

Remember:
CTR > 0.5 in PA view = Cardiomegaly

الصورة دي عبارة عن erect chest radiograph بتوضح وجود Dextrocardia.

في الحالة دي القلب بيكون موجود بالكامل في الـ right hemithorax بدل الـ left، مع اتجاه الـ cardiac apex ناحية اليمين.

في الأشعة هنلاحظ:
• Cardiac silhouette في الجهة اليمنى من الصدر
• Apex موجه ناحية اليمين (rightward pointing apex)
• عادةً موقع المعدة (gastric bubble) بيكون في الناحية المقابلة حسب نوع الترتيب (situs)
• ممكن يترافق مع mirror image of normal cardiac orientation

مهم جدًا التفريق بين:
• Dextrocardia (true right-sided heart position)
vs
• Dextroposition (heart displaced لليمين بسبب سبب خارجي زي lung collapse أو pleural effusion)

Diagnosis: Dextrocardia

Remember:
Heart apex pointing to the right + cardiac shadow in right hemithorax = Dextrocardia

الصورتان دول لنفس المريض، وبيظهر فيهم وجود Septal lines متعددة في الـ left lower lung zone.

الـ septal lines عبارة عن خطوط دقيقة جدًا تظهر بالقرب من pleural surface، وتمتد للداخل بزاوية قائمة تقريبًا على chest wall.

في الصورة الأصلية قد تكون الخطوط subtle وصعبة الملاحظة، لذلك في الصورة على اليمين تم تحديد 3 من الـ septal lines باستخدام الأسهم الصفراء.

وجود هذه الخطوط يدل على thickening of the interlobular septa، وغالبًا يحدث بسبب interstitial fluid accumulation.

أكثر سبب شائع:
Pulmonary interstitial oedema في مرضى heart failure.

ومن الأسباب الأخرى:
• Lymphangitic carcinomatosis
• Pulmonary fibrosis

Diagnosis: Septal Lines

Remember:
Short fine peripheral lines perpendicular to pleura = Septal lines
وغالبًا تشير إلى Interstitial Pulmonary Oedema.

الـ septal lines تظهر كـ very fine linear opacities عند peripheral lung، وتمتد perpendicular على pleura أو chest wall بزاوية 90°.

الخطوط دي تنتج عن thickening of the interlobular septa، وغالبًا تكون subtle جدًا لذلك من السهل عدم ملاحظتها أثناء قراءة الأشعة.

في الصورة على اليمين تم تحديد 3 من الـ septal lines بالأسهم الصفراء لتوضيح شكلها بشكل أفضل.

أشهر سبب لظهور الـ Septal lines:
Interstitial pulmonary oedema نتيجة heart failure أو pulmonary venous hypertension.

Diagnosis: Septal Lines (Kerley lines)

Remember:
Fine peripheral lines perpendicular to pleura = Septal lines
وغالبًا تكون علامة على Interstitial Pulmonary Edema.

الـ septal lines تظهر كـ very fine linear opacities عند أطراف الرئة، وتمتد perpendicular على pleura أو chest wall بزاوية 90°.

الخطوط دي تنتج عن thickening of the interlobular septa بسبب interstitial fluid overload.

في الصورة على اليمين تم تحديد الـ septal lines باللون الأصفر لتسهيل رؤيتها، لأنها غالبًا subtle جدًا وسهلة الـ missed detection.

وجود Septal lines في مريض heart failure يشير غالبًا إلى:
Interstitial pulmonary oedema.

Diagnosis: Septal Lines (Kerley B lines)

Remember:
Fine peripheral lines perpendicular to pleura = Septal lines
وغالبًا تكون علامة على Interstitial Pulmonary Edema secondary to Heart Failure.

Septal lines

الـ Septal lines هي خطوط رفيعة جدًا تظهر في أشعة الصدر نتيجة امتلاء أو thickening للـ pulmonary interlobular septa بالـ fluid أو tumor أو fibrosis.

في الطبيعي الـ interlobular septa لا تُرى في الـ chest X-ray، لكن عندما يحدث بها thickening تبدأ في الظهور كـ fine linear opacities عند أطراف الرئة.

في الأشعة هنلاحظ:
• خطوط رفيعة جدًا عند peripheral lung
• تمتد للداخل من pleural surface
• تكون perpendicular على pleura بزاوية 90°
• غالبًا يسهل missed detection لذلك لازم actively look for them

قديمًا كانت تُعرف باسم:
• Kerley A lines → في central lung
• Kerley B lines → في peripheral lung

لكن حاليًا يفضل استخدام مصطلح:
Septal lines

أشهر الأسباب:
• Interstitial pulmonary oedema نتيجة pulmonary venous hypertension أو heart failure
• Lymphangitic carcinomatosis بسبب انتشار الورم داخل الـ lymphatics
• Fibrosis in pneumoconiosis (rare)

معلومة مهمة:
في حالات malignancy قد تكون الـ septal lines bilateral وقد يصاحبها hilar lymph node enlargement.

Diagnosis: Septal Lines

Remember:
Fine peripheral lines perpendicular to pleura = Septal lines
وأشهر سبب لها هو Interstitial Pulmonary Edema.

في هذه الحالة الـ shadowing كان:
🔹 Bilateral
أي موجود في كلا الجانبين (right + left).

📍 نلاحظ:

Ill-defined opacities تمتد من الـ hilar regions في كلا الرئتين

تمتد جانبيًا لكنها تتوقف قبل حواف الرئة بحوالي: 📏 3–4 cm

🩷 في الصورة اليمنى: تم تحديد Bat’s wing pattern باللون الوردي.

⚠️ ملاحظة إضافية:

🔸 وجود calcified pleural plaques
منتشرة في كلا الحقلين الرئويين

📌 وهذا قد يشير إلى:

previous asbestos exposure (غالبًا)

🦇 Bat’s Wing Pattern Shadowing

في حالات الـ acute pulmonary oedema قد يظهر شكل مميز بالأشعة يسمى:

🦇 “Bat’s wing pattern”
وهو عبارة عن:

Ill-defined opacities
(عتامات غير واضحة الحدود)

قد تكون: • Bilateral
• أو Unilateral

وتكون موجودة في: 📍 المنطقة المركزية للرئتين
(Perihilar region)

وتمتد جانبيًا لكن تتوقف قبل حواف الرئة بحوالي: 📏 2–3 cm

✨ شكل الـ opacity يشبه جناحي الخفاش، لذلك سُمّي: 🦇 Bat’s wing appearance

⚠️ حتى الآن لا يوجد تفسير واضح لهذا التوزيع المميز، لكنه يحدث غالبًا في مرضى الـ pulmonary oedema.

🔗 يرتبط بشكل كلاسيكي مع:

Left ventricular failure ⬇️
Pulmonary oedema ⬇️
Bat’s wing appearance on chest X-ray

🦇 Unilateral Bat’s Wing Pattern

في هذه الحالة الـ shadowing كان:
🔹 Unilateral
أي موجود في جهة واحدة فقط (الجهة اليسرى).
📍 نلاحظ وجود:

Ill-defined opacity على الجانب الأيسر
وتمتد جانبيًا لكنها تتوقف قبل حواف الرئة بحوالي: 📏 2–3 cm
🩷 في الصورة اليمنى: تم تحديد الـ “Bat’s wing” pattern باللون الوردي.
⚠️ يمكن أيضًا ملاحظة:
🔸 Middle lobe consolidation
🔸 علامات أخرى للـ pulmonary oedema مثل:
Lower zone symmetrical diffuse fuzzy shadowing
