The magic of DNM, yesterday I reacived a gentleman who experienced a very sharp pain from gym exercises, the pain was below the right shoulder blade but with difficult even turning. It was really sharp pain, just positioned him in side lying on the left side, bend the knees to around 70degress and did the skin stretch for nearly 10minutes. Did 3 sets of the stretch but there was immediate relief and pain reduced considerably, turning became much easier and could not believe I did have to do the traditional massage he expected but was able to change the pain experience and improved thoracic mobility. At this point I believe manual therapy that we were taught in physio schools needs to be redifined Diane Jacobs Mutiibwa Allan Dermoneuromodulating

Secondary compensations will always distract us from the primary driver responsible for your pain.

The pain you are experiencing could be as a result of a structural problem(joint, ligament, muscle, receptors or vestibular), physiological problem (organ, metabolic load or visceral) or psycho-emotional problems(trauma imprint, unresolved stress, threat perception, association and emotions)

Yesterday, I received a 47-year-old patient complaining of a burning sensation at the front of the knee, only felt after prolonged hours of sitting in a chair. It had been ongoing for nearly a year and he felt it was getting worse by the day. Following provocative maneuvers, it was evident that this had nothing to do with the deeper underlying tissues; I suspected cutaneous nerve irritation around the knee. I did a lot of pain education and explained how the brain can get over-sensitized and the need to down-regulate it. He was okay with me trying something he had never heard of, Dermoneuromodulating. I took him through the supine lying position and my favorite technique, ballooning, for close to six minutes and asked him to sit again for at least 30 minutes, since symptoms would start manifesting at around the same time; no issues arose. He crossed his leg while sitting and there were no issues. I asked him to monitor the knee and provide feedback. I will continue the therapy next week, incorporating knee exercises.

According to The Sports Physio patients are less concerned with your vast knowledge about tissue and the human, all that matter to them are what it is, is it serious, how do I go about it, how long will it take and wether one can still work, train, sleep or function.

Most people breathe, but few breathe correctly. In physiotherapy, I have discovered a crucial principle: improving someone's breathing can fundamentally alter their bodily functions. During a simple inhale, the diaphragm contracts and descends, pressure builds in the abdomen, the abdominal wall and pelvic floor respond, the spine subtly extends and stabilizes, and deep muscles like the multifidus provide support and control. This process is not merely breathing; it is coordination. When this system functions optimally, the spine is supported, movement becomes efficient, stress decreases, and pain subsides. Conversely, poor breathing leads to core instability, unnecessary spinal loading, pain, stiffness, and persistent bodily tension. Over the years, I have helped numerous patients alleviate pain, enhance movement, and improve overall well-being by focusing on breathing techniques rather than exercises. Breathing is not solely about oxygenation; it is the foundation of movement, stability, and recovery. If you are struggling with back pain, stress, or stiffness, it may be beneficial to examine your breathing patterns. 🫁 Rolian Physiotherapy Services – Kampala for Breath training, Movement and Function will forster Real recovery
