Drs. Fanning, Tannous, Ferrer, Remus, and Kolbe recently attended a medical education session focused on helping teens with chronic conditions transition to adults managing their own healthcare.
Here are a few ways families can start building those skills at home during the teen years:
✔️ Know your condition
Be able to name your medical condition(s) and give a brief summary.
✔️ Understand your medications
Know the names, doses, and when to take them. Consider keeping this info in a “medical passport” or secure note on your phone.
✔️ Stay connected to your care team
Know the names of your doctors and therapists. Know how to contact them, access your patient portal, and schedule appointments.
✔️ Be prepared for visits
Carry your health insurance card and practice filling out medical forms and questionnaires.
✔️ Plan for future support if needed
For teens who may need continued support into adulthood, explore legal options like supported decision-making or guardianship.
