🫁 Promising results for a new COPD biologic: Tozorakimab

Two phase 3 randomized trials, OBERON and TITANIA, published in NEJM evaluated tozorakimab in 1,750 patients with COPD and exacerbations despite maintenance inhalers.

🔬 What is it?
An investigational monoclonal antibody targeting interleukin-33 (IL-33), a signaling protein involved in airway inflammation.

💉 Trial dose: 300 mg subcutaneously every 4 weeks for 52 weeks, added to usual inhaled treatment.

📉 Moderate-to-severe exacerbations decreased:
• Former smokers—the primary analysis: 29% in OBERON and 34% in TITANIA.
• Overall population, including current and former smokers: 30% and 29%, respectively.

In the overall population, annual exacerbation rates fell from approximately 2.0 to 1.4 per patient.

🧪 No minimum blood eosinophil count was required for enrollment. However, this does not mean the drug is appropriate for every patient with COPD.

⚠️ Overall adverse-event rates were broadly comparable, but the accessible abstract does not provide mortality or serious-adverse-event counts; a survival benefit cannot be claimed.

The treatment remains investigational and under regulatory review. These results do not yet establish routine prescribing eligibility.

🧠 Promising treatment for recovery after ischemic stroke

A randomized trial published in JAMA on September 10, 2026 evaluated loberamisal, an investigational small-molecule drug designed to protect brain cells and support recovery after stroke.

🔬 How does it work?
It targets two pathways:
• Disrupts the PSD-95–nNOS interaction, aiming to reduce excessive nitric oxide production and injury from overstimulated nerve cells.
• Enhances α2-containing GABA-A receptor activity, strengthening inhibitory signaling that helps calm excessive neuronal activity.

These proposed mechanisms are supported mainly by preclinical research.

The LAIS trial randomized 998 patients to loberamisal 40 mg IV daily for 10 days or placebo, starting within 48 hours, alongside standard care.

At 90 days:
✅ Minimal or no disability: 69.7% versus 56.3%—about 13 additional patients per 100 treated.
📊 Relative risk: 1.24 (95% CI 1.12–1.36).
⚠️ Serious adverse events: 8.6% versus 10.7%.
Deaths: 6 versus 10—insufficient to establish a survival benefit.

Encouraging findings, but this China-only trial excluded thrombectomy patients. Further confirmation is needed before routine use; the drug should not replace or delay reperfusion treatment.

Adult Pneumococcal Vaccination: Updated ACIP/CDC Recommendations

Pneumococcal vaccination is now recommended for all adults age 50 years and older who have not previously received a pneumococcal conjugate vaccine, or whose vaccination history is unknown. ACIP expanded the routine adult age-based recommendation from ≥65 years to ≥50 years in October 2024. (CDC)

Adults age 19–49 years should also receive pneumococcal vaccination if they have certain risk conditions, including immunocompromising conditions, cochlear implant, CSF leak, chronic heart/lung/liver disease, diabetes, alcoholism, or cigarette smoking. Hypertension alone is not a qualifying risk condition. (CDC)

Current adult vaccine options include:

1. PCV20 — single dose; series complete
2. PCV21 — single dose; series complete
3. PCV15 followed by PPSV23 — PPSV23 is typically given ≥1 year later, or ≥8 weeks later for patients with immunocompromising conditions, CSF leak, or cochlear implant. (CDC)

For adults who previously received PCV13 and/or PPSV23, additional vaccination may still be indicated depending on age, prior vaccine type, timing, and risk factors. In some adults ≥65 years who already completed PCV13 plus PPSV23, shared clinical decision-making may support PCV20 or PCV21 at least 5 years after the last pneumococcal vaccine. (CDC)

Key takeaway: Pneumococcal vaccination is no longer just a “65 and older” discussion. For adults ≥50, and for younger adults with qualifying risk conditions, clinicians should review pneumococcal vaccine history and offer an updated PCV-based regimen when indicated.
