Responsibilities and Duties LPN’s
• Follow the plan of care initiated for patients
• Provide nursing care, which may include but is not limited to physical assessment, medication administration, wound care, IV therapy, tube feeding, trach, vent and comfort care. .
• Regularly communicate patient care needs and progress to each patient’s physician.
• Supervise personal home care aides as necessary.
• Provide care to patients following home care procedures and strategies to keep the patient safe at home.
• Remain in compliance with HIPAA guidelines as well as all state and federal regulations.
• Document clinical notes in a clear and concise manner.
Qualifications and Skills
• Must be a LPN in Georgia (or hold compact state licensure)
• CPR certification
• At least 6 months of experience as an LPN
• Trach/vent experience preferred
• Reliable transportation
• Valid driver’s license and insurance
• Availability to work on weekends, evenings, and holidays
• Prior home care experience preferred

• $29 per hour
