Adult children are often left trying to coordinate an aging parent’s care across cardiology, neurology, primary care, and several other providers.

Each doctor may be doing their part—but no one is seeing the whole picture.

A new medication may conflict with something another provider prescribed. Test results stay buried in one portal. A change in symptoms gets mentioned at one appointment but never reaches the next doctor. Meanwhile, the family is left wondering what matters, who needs to know, and what should happen next.

This isn’t just an appointment-scheduling problem. It’s a clinical coordination problem.

I work with elders and families across Connecticut as a geriatric care manager, and right now I'm helping a family in Vernon find a homemaker/companion caregiver who is fluent in American Sign Language.
We need someone patient, dependable, and comfortable communicating in ASL to help with companionship and light household support.

That's often appropriate.

“Cleared to go home” is not the same as “safe at home.”

When the hospital says your mom, dad, spouse, or loved one is medically stable, that usually means:

→ Vitals are in range
→ The acute issue has been treated
→ The hospital is no longer the right level of care

That is an important decision.

But safe at home is a different question.

Safe at home depends on whether the real-life pieces have actually been figured out:

→ Is this confusion new, or was it already there before the hospital?
→ Were oxygen levels checked while walking, not just lying in bed?
→ Does the medication list match what is actually in the house?
→ Can they safely get to the bathroom, eat, sleep, and move around at home?
→ Are follow-up appointments scheduled — and actually reachable?
→ Does the family understand what changed and what to watch for?

Hospital case managers help confirm that there is somewhere to go and basic services are in place.

But having a destination is not the same as having a plan that works once your loved one gets home.

And that is where many families get overwhelmed.

You leave with a folder, a phone number, a printout, and the responsibility of figuring out the rest yourself.

If you have ever sat at the kitchen table after a hospital discharge thinking, “I don’t even know what I’m supposed to do next,” you were not being dramatic.

You were holding a care management gap.

That gap can look like:

→ Medication bottles that do not match the discharge paperwork
→ Follow-up appointments that are weeks away
→ New confusion that no one clearly explains
→ Weakness, falls, oxygen issues, or behavior changes that show up at home
