Every practice has a waitlist and every practice has slots that quietly go unfilled. Usually those two facts are connected by a block nobody cleared.

A provider cancels a 2pm commitment. It leaves their calendar immediately. In athenahealth the time is still held, so it does not appear as available, so the front desk never offers it to anyone. Two hours that could have gone to a patient who has been waiting three weeks.

Delete the appointment on whichever calendar you are already in. The hold comes off athenahealth in the same moment and the slots go back on the schedule. Nobody has to send a message saying the 2pm opened up.

The cancellation still happens. It just stops costing you the appointment that could have replaced it.

Here is a small thing that costs practices real money.

Someone reschedules a meeting from Tuesday to Thursday. On the calendar they moved it in, it's done. In athenahealth, Tuesday is still blocked, because the hold was created once and nothing told it to move.

That Tuesday time is now free and unbookable at the same time. It stays that way until somebody notices, which is usually never, because nobody is auditing last week's blocks.

Move the appointment wherever you happen to be working. Outlook, Google Calendar, athenahealth. The old slots reopen, the new time closes, and the schedule reflects what is actually true.

Ask whoever runs your front desk how many calendars they check before booking a patient. The answer is usually two, sometimes three.

That is the actual cost of these systems not talking. Not the software, the habit it forces on people. Someone enters a staff meeting in Microsoft 365, then enters it again so the EHR knows the time is gone. It works until somebody is busy and skips the second step, and then a patient gets booked into a slot that was never available.

Enter it once, anywhere. Outlook, Google Calendar, or athenahealth. The other two have it.

If your practice runs on Microsoft 365, you already know the gap.

The staff meetings are in Outlook. The vendor calls are in Outlook. The interviews, the committee time, the block someone reserved three weeks ago. All in Outlook. And athenahealth has never had any idea any of it exists.

So the front desk books a patient into a slot that was spoken for, and nobody finds out until somebody is standing in the lobby.

This video shows both directions working. A meeting created in Outlook blocks the time in athenahealth. A patient appointment booked in athenahealth appears on the Outlook calendar. Reschedule either one and both move. Cancel either one and both clear, with the time reopening for booking.

A question we get from physicians more than any other: does my practice see everything on my personal calendar?

No, and you control exactly where that line sits.

Private keywords are words you choose. Put one in a Google Calendar event title, like PRIVATE or PERSONAL, and that event never reaches athenahealth. It stays entirely on your side.

It matters more than it sounds like it should. A therapy appointment, a job interview, a specialist you are seeing yourself. You may want the time blocked without anyone at the practice knowing what it is for. This is how you do that without keeping a second calendar.

Creating a meeting is the easy part. Moving one is where integrations usually show their limits.

The common failure is that the new time gets blocked and the old block stays put. Nobody notices right away. A month later the schedule has holds on it for meetings that moved weeks ago, and somebody has to go clean them up by hand.

This shows a reschedule across all three calendars. Change it in Outlook, Google Calendar, or athenahealth, and the other two follow. The blocker moves, the old slots open back up, and the new ones freeze.

Whichever calendar you happened to make the change in is the right one.

New one, and it starts somewhere different.

Most practices we talk to run Microsoft on the administrative side. Outlook is where the staff meetings, the vendor calls, and the interviews live. Google Calendar is where personal commitments go. And athenahealth has no idea about either.

Now it does. Create a meeting in Outlook, it syncs through to Google Calendar, and a Sporo blocker goes up on the athenahealth schedule with the slots underneath it frozen.

One entry, three calendars, no double entry and no copying things over on Monday morning.

Same demo as last time, opposite setting.

A meeting on Google Calendar during clinical hours puts a blocker on the athenahealth schedule and freezes the slots under it. Forty five minutes over 15 minute slots means three slots frozen, plus a labeled hold anyone can see.

Then watch me drag it to an afternoon with no clinical schedule. The blocker is still there. Nothing to freeze, but the time is held either way.

That is the whole difference between the two settings. Block slots only works on the template you already built, so unscheduled time stays open. Blockers hold the time no matter what is underneath.

Neither is the right answer for everyone. It depends on whether your staff need to see held time before the schedule exists.

Easier to show than explain, so here it is in one take.

A meeting on Google Calendar freezes the athenahealth slots it covers. Forty five minutes over 15 minute slots means three slots freeze.

Then watch me drag the same meeting to an afternoon with no clinical schedule. Nothing happens at all. No slots there, so nothing to freeze. Drop one clinical slot into that time and it freezes on the spot.

Block slots only works on the template you already built. It doesn't add to it. Your schedule stays exactly as you set it up, and time with no slots on it stays open.

If you'd rather hold unscheduled time too, that's what the blocker setting is for.
