With Amref Health Africa, trained local Village Health Leaders to bring essential supplies, counseling, and safety protocols directly to the doorsteps of pregnant women in rural . Here, where facility-based deliveries hovered near 15%, safe and skilled delivery rates more than doubled. Zero maternal fatalities were recorded across 38 geographically isolated communities since intervention inception.

Average coverage can mask local gaps. A new Gavi VaccinesWork blog from the JSI-led Zero-Dose Learning Hub explores how Lot Quality Assurance Sampling (LQAS) can support more targeted, actionable approaches to identifying and reaching zero-dose and under-immunized children.

In immunization programs, national and district estimates of coverage are often used to track progress. But those averages can conceal variation at subdistrict levels, where children are still being missed. LQAS helps program managers move beyond the question, “What is coverage?” and ask, “Where is coverage failing?” As a classification-based survey method, LQAS supports decentralized decision-making by helping identify where vaccination coverage may be falling below target and where corrective action may be needed.

JSI supports safety-net providers in translating complex payment reform policies into practical strategies for success in value-based payment. We are conducting an evaluation of the Episcopal Health Foundation’s investment in 11 Texas community health centers to build capacity for population health and value-based payment design. We use a mixed-methods approach that involves longitudinal assessments, key informant interviews, and annual site visits. This work identifies the strengths, needs, and strategies clinics use when transitioning to value-based models.

💫Mission accomplished! JSI recently supported the Mozambique Expanded Programme on Immunization (EPI) with printing and distributing 10.6 tons of vaccine record tools, including health cards, daily and monthly vaccination summaries, stock control sheets, and more, to the provinces of Nampula, Zambézia, Sofala, and Manica.

This collection of tools closes a huge gap by replacing informal and ad hoc record-keeping with standardized vaccination records. This standardization improves data accuracy across various information systems and enables health care workers, program managers, and senior leaders to make strategic decisions based on reliable evidence. This means they are better equipped to identify and prioritize children and communities that need vaccination services the most.

JSI thanks the Ministério da Saúde - MISAU, the Provincial Health Directorates, and all those involved who made this shipment possible. Together, we continue to strengthen health systems to achieve better health outcomes for all.

💫Missão cumprida! A JSI apoiou recentemente o Programa Alargado de Vacinação (PAV) de Moçambique com a impressão e distribuição de 10,6 toneladas de instrumentos de registo de vacinação para as províncias de Nampula, Zambézia, Sofala e Manica.

Estes simples formulários preenchem uma enorme lacuna, substituindo o registo informal e improvisado por registos de vacinação padronizados. Esta padronização melhora a precisão dos dados em diversos sistemas de informação e permite que os profissionais de saúde, gestores de programas e líderes tomem decisões estratégicas com base em evidências fiáveis. Most cases of severe maternal morbidity and mortality could be prevented with timely care during pregnancy, childbirth, and the postpartum period. However, across the U.S., access to perinatal care is becoming increasingly difficult—especially in rural areas—given a large number of birth facility closures. A recent study conducted by JSI on behalf of the Colorado Department of Public Health and Environment investigated the consequences of these closures and their impact on patients and found that these closures lead to increased travel time and financial burden for patients.

We found that not only do birth facility closures have a major impact on patients, they can also shift the burden of obstetric emergencies to hospital emergency departments and emergency medical services in the impacted area, which are often underprepared to handle them.

The study findings highlight the need to strengthen system and workforce readiness for obstetric emergencies amid gaps in perinatal care access. We explore JSI’s recommendations for strengthening Colorado’s perinatal system of care and ensuring access to safe, reliable, quality care, as well as implications for obstetric emergency care more broadly: https://www.jsi.org/insights/strengthening-perinatal-care-support-obstetric-emergency-readiness/
