Independence isn't the absence of support.

“I live by myself and am glad that I can call if I need anything at any time. I'm comforted knowing they are always there for me and appreciate their service.”

That's a CareTrack patient describing what support between visits actually feels like.

September is Healthy Aging Month, and most of the conversation will be about appointments, screenings, and prevention. All of it matters. But an older adult managing two or three chronic conditions spends only a few hours a year inside a physician's office. The rest of the time, they're at home, managing on their own.

Healthy aging depends on what's available during those hours. Someone to call. A care coordinator who already knows the history. A question answered on a Tuesday afternoon instead of held for three months.

That's the work CareTrack does on behalf of the practices we support — care coordination, patient outreach, and around-the-clock access, documented in the physician's own EHR, so the patient stays connected to the care team they already trust.

Independence isn't the absence of support. It's having support you don't have to go looking for.

Patient advocacy doesn't stop when an appointment ends.

It continues through every follow-up, every answered question, every care plan, and every conversation that helps patients stay connected to their providers.

Today we celebrate everyone who advocates for patients every day, both in front of and behind the scenes.

Behind every successful patient experience is a team of professionals keeping healthcare organized, connected, and moving forward.

During Health Unit Coordinators Week, we recognize the coordinators, patient navigators, referral specialists, schedulers, and administrative professionals whose work keeps communication flowing and patients supported.

Their contributions often happen behind the scenes, but they make exceptional care possible!

What does healthy aging look like between doctor visits?

Sometimes, it's as simple as replacing uncertainty with peace of mind.

One patient participating in Remote Patient Monitoring (RPM) through CareTrack shared:

“Before RPM, I only had my blood pressure checked when I came to the doctor. Now I check it every morning, and if I feel bad, I can check it again. I don’t have to guess whether my blood pressure is up or down. It gives me peace of mind knowing my care team and doctor can see my readings.”

On National Senior Citizens Day, we're celebrating older adults and recognizing what an aging population truly needs from healthcare.

Healthy aging isn't simply about more appointments. It's about helping people remain independent, engaged, and connected to their care throughout the other 99% of their lives.

For older adults managing chronic conditions, programs like Remote Patient Monitoring can provide greater visibility into their health at home while keeping them connected to the care teams and physicians they already know and trust.

Because technology should never make healthcare feel more complicated. At its best, it can help make patients feel more informed, more confident, and more connected to their care.

A blood pressure reading doesn't improve a patient's health. What happens next does.

The best care management programs pair technology with trained care teams who review trends, document interventions, communicate with patients, and escalate concerns according to physician-defined workflows.

Behind every alert should be a process, not just a notification.

When health information matters most, access and communication matter.

August is MedicAlert Awareness Month, highlighting the importance of making critical medical information accessible when patients may not be able to communicate it themselves.

That same principle, having the right information available at the right time, is essential to proactive care!

For patients managing chronic conditions, meaningful health changes don't always happen during an office visit. Remote Patient Monitoring (RPM) can give care teams greater visibility into what's happening between appointments, while ongoing care management helps keep patients connected, supported, and engaged in their health.

At CareTrack, we help practices extend care beyond the exam room through RPM, Chronic Care Management (CCM), Principal Care Management (PCM), Advanced Primary Care Management (APCM), and Annual Wellness Visit (AWV) support, with care activity documented within existing EHR workflows.

Because better-informed care can start with something as simple as having the right information at the right time.

CMS's proposed CY 2027 Medicare Physician Fee Schedule includes potential changes to Remote Patient Monitoring (RPM), including how clinical staffing may be structured for Medicare reimbursement. But there's an important distinction healthcare organizations should keep in mind:

This is a proposed rule, not a final rule. CMS is accepting public comments through September 14, 2026.

Whether you're a current CareTrack partner or simply evaluating what these proposed changes could mean for your organization, our team is available to help you understand the proposal, identify relevant program data, and think through next steps.

We're actively evaluating the proposed changes and developing contingency models designed to preserve patient access and support compliant RPM delivery if changes are ultimately finalized.

CareTrack has long been at the forefront of helping healthcare organizations build sustainable, compliant remote care programs, and we'll continue helping lead the conversation as the industry evolves. Our experience spans Remote Patient Monitoring (RPM), Chronic Care Management (CCM), Annual Wellness Visits (AWV), Advanced Primary Care Management (APCM), and other connected-care strategies, giving healthcare organizations a knowledgeable partner prepared to help navigate both today's requirements and what's coming next.

For now, our message to healthcare organizations is simple: Don't make operational decisions based on a proposed rule. Prepare, ask questions, and stay engaged in the process.

CMS is accepting comments through September 14, and provider perspectives matter. We encourage healthcare organizations to review the proposal, consider how it could affect their patients and care models, and participate in the comment process.

If your organization would like help understanding the proposal, identifying relevant data from your own program, or preparing a comment, reach out to the CareTrack team. We're happy to help.

No two healthcare organizations face the exact same challenges.

Some practices are looking to improve chronic disease management. Others want to expand preventive care, better engage Medicare patients, streamline workflows, or create new recurring revenue without increasing headcount.

That's why CareTrack offers a comprehensive suite of care management solutions designed to work together, and inside your existing EHR.

Whether your organization is focused on:

✔ Remote Patient Monitoring (RPM) to monitor patients between visits and identify concerns earlier.

✔ Chronic Care Management (CCM) to keep patients with multiple chronic conditions engaged and supported throughout the month.

✔ Principal Care Management (PCM) for patients managing a single complex chronic condition.

✔ Advanced Primary Care Management (APCM) to expand care management across a broader Medicare population with simplified reimbursement.

✔ Annual Wellness Visits (AWVs) to increase preventive care, identify care gaps, and create opportunities for ongoing patient engagement.

CareTrack provides the people, workflows, and technology to help practices deliver better care while reducing administrative burden.

The goal isn't simply offering more programs.

It's helping healthcare organizations build a connected care strategy that improves patient outcomes, supports providers, and strengthens long-term practice performance.

Which area is your organization most focused on today: preventive care, chronic disease management, workflow efficiency, patient engagement, or practice growth? We'd love to hear from you!

Most organizations celebrate enrollment numbers, devices shipped, or readings collected. But are those the right metrics?

The most successful care management programs focus on what happens after enrollment: improved patient outcomes, timely interventions, engaged patients, streamlined workflows, and meaningful clinical impact.

When evaluating any Remote Patient Monitoring or Chronic Care Management program, ask yourself: What does success actually look like for your organization?

Healthcare leaders: Which KPI matters most to your team today—patient outcomes, operational efficiency, reimbursement, or patient engagement?
