Independence isn't the absence of support.

That's a CareTrack patient describing what support between visits actually feels like.
September is Healthy Aging Month, and most of the conversation will be about appointments, screenings, and prevention. All of it matters. But an older adult managing two or three chronic conditions spends only a few hours a year inside a physician's office. The rest of the time, they're at home, managing on their own.
Healthy aging depends on what's available during those hours. Someone to call. A care coordinator who already knows the history. A question answered on a Tuesday afternoon instead of held for three months.
