Case Manager Job Responsibilities:

Accomplishes clients' care by assessing treatment needs; developing, monitoring, and evaluating service plans and progress.

Case Manager Job Duties:
-Monitors service plan monthly
-Completes re-evaluations
-Completes quarterly home visits
-Documents case management in EHR system
-Coordinates services for clients
-Attends policy meetings
-Attends mandatory trainings.

Case Management Skills:
-Must be able to use EHR System
-Excellent oral and written communication skills required

Education
-Must be a licensed Social Worker; or have a Bachelor’s degree or Master’s degree with at least two years of assessment and care planning experience with clients.
