Cherokee Indian Hospital Authority is committed to making sports physicals as simple, convenient, and accessible as possible for our families while ensuring every student-athlete receives the safe, high-quality care they deserve.

That's why we've introduced a new Sports Physical Scheduling Process designed to streamline appointments, reduce confusion, and help families get the forms they need as quickly as possible.

If your child has completed a well-child exam within the last 12 months, simply complete your portion of the sports physical form (https://ow.ly/CwFt50ZlJya) and submit it to your Primary Care Team. Our providers will complete the medical portion within 48 hours whenever possible, and we'll notify you when it's ready for pickup.

If your child is due for their annual well-child exam, we'll schedule a combined well-child and sports clearance appointment. If your primary care provider isn't available within the needed timeframe, we'll prioritize getting your child in with one of our same-day providers to help avoid unnecessary delays.

This improved process allows us to maximize appointment availability, reduce wait times, and ensure every athlete receives a thorough medical evaluation before stepping onto the field, court, or track. It also helps identify any health concerns that may require additional evaluation so we can keep our young athletes healthy and safe throughout the season.

Our goal is to make getting a sports physical as easy and accessible as possible while never compromising on patient safety. We encourage families to schedule well-child visits early each year so sports clearance can be completed quickly when the season arrives.

Thank you for trusting Cherokee Indian Hospital Authority to care for your student-athletes. We look forward to helping them have a healthy, successful season!

We want to make our patients, community, and partners aware of a planned telephone system upgrade taking place on Wednesday, July 8, between 11:00 a.m. and 1:00 p.m. EST.

During this time, Cherokee Indian Hospital Authority will be transferring our phone numbers to a new telecommunications provider. This proactive upgrade is part of our ongoing commitment to ensuring our phone system remains dependable, resilient, and available when our patients need us most.

As a healthcare organization, reliable communication is essential. This change is being made to strengthen the reliability and redundancy of our phone services, helping us better serve our community now and in the future.

During the scheduled two-hour transition window, callers may experience intermittent disruptions. Phone lines may temporarily go in and out of service, callers may occasionally receive a busy signal, and some outbound calls may also be affected while the transfer is completed.

Our Information Technology team and telecommunications partners will be closely monitoring the transition to minimize any disruption and restore full service as quickly as possible.

Thank you for your patience as we continue investing in the infrastructure that helps us provide safe, reliable care to our community.
