A heavy lid is not always “just extra skin.”

That is the part of eyelid surgery most people never hear in a consult.

Upper-lid heaviness can come from extra skin, a low brow, eyelid position, or all three. Under-eye bags can come from fat pads, loose skin, hollowing, or the cheek beneath them. If you only treat one fold, the eyes can look different—not rested.

In her latest blog, Dr. Judith Gurley walks through what actually makes upper and lower blepharoplasty look natural: planning the whole eye area, not taking as much tissue as possible. That may mean upper lids, lower lids, both (quad blepharoplasty), or adding support when volume is the real issue.
