Who is eligible for Habitat Health?

To qualify for Habitat Health PACE, you must:
1. Be 55 or older
2. Live in a covered ZIP code (you can check this on our website)
3. Be able to live safely in your community at the time of enrollment.
4. Be certified by the State as meeting the nursing home level of care. (You don't need this to apply, we will help with this part.)

Most participants are eligible for both Medicare and Medi-Cal. Not sure if you qualify? Our team will help you find out.

At Habitat Health, participants don't just attend activities. Sometimes they teach them.

At our South LA summer party, one of our participants shared the beauty of her culture through music and movement with a Hawaiian dance lesson. No experience needed. Everyone was welcome to join the fun.

There were leis, coloring, cake, and a room full of people learning something new together.

This is what a day at Habitat Health can look like. Participants don't just come here for appointments. They participate in creating the center experience, bringing their stories, skills, and the traditions they grew up with.

In case you missed it...

This week we celebrated the grand opening of haircut services at our South LA center!

For many PACE participants, getting to a salon or barbershop is no longer easy. Bringing this service to the center means providing one more way our participants can look and feel their best — without having to worry about coordination, transportation, or cost.

We know that feeling well-groomed is directly tied to self-esteem, social connection, and emotional well-being. This is what whole-person care looks like in practice.

We're thrilled to announce the grand opening of Habitat Health haircut services — a new service for our participants, provided right here at the center.

For many, getting to a salon or barbershop is not easy. Having this service available at the center means one less thing to worry about — and one more reason to feel like themselves.

This week our superheroes wore capes!

Superhero Day at Habitat Health South LA was a reminder of something we already knew: the people who care for our participants are extraordinary. It's not just about the skills and training they bring to our participants, but also genuine warmth, patience, and enthusiasm they bring to the center every single day.

To our center teams: thank you. The work you do matters more than words can say. Our participants are fortunate to have you in their corner.

Also, next time please take video when you do your "fly" around the building. That's the kind of stuff we need to see. 😆
