Kramer Advocacy LLC is seeking input from families who have experienced challenges involving hospital discharge planning, long-term care placement, skilled nursing placement, guardianship concerns, or transitions of care for elderly or medically vulnerable loved ones.
As part of a community needs assessment, we are gathering information about family experiences to better understand barriers, challenges, and gaps in services.
Have you experienced any of the following?
• Concerns about an unsafe or premature discharge• Difficulty obtaining skilled nursing placement• Insurance barriers affecting care• Lack of communication from providers• Exclusion from discharge planning discussions• Difficulty accessing community resources after discharge• Concerns about patient safety during transitions of care
If so, we would like to hear your story.

Were appropriate services available when needed?
