Kramer Advocacy LLC is seeking input from families who have experienced challenges involving hospital discharge planning, long-term care placement, skilled nursing placement, guardianship concerns, or transitions of care for elderly or medically vulnerable loved ones.

As part of a community needs assessment, we are gathering information about family experiences to better understand barriers, challenges, and gaps in services.

Have you experienced any of the following?

• Concerns about an unsafe or premature discharge• Difficulty obtaining skilled nursing placement• Insurance barriers affecting care• Lack of communication from providers• Exclusion from discharge planning discussions• Difficulty accessing community resources after discharge• Concerns about patient safety during transitions of care

If so, we would like to hear your story.

Please comment below, send a private message, or answer the following questions:

Were you included in discharge planning decisions?

Did you feel your loved one was discharged safely?

Did insurance or financial issues affect care?

Were appropriate services available when needed?

What was the biggest challenge your family faced?

What changes would have improved the experience?

All responses may be used in aggregate form to identify community needs and advocate for improvements in care transitions and family support services.

I have been skipping my self care of just being, just sitting, just relaxing, just breathing, just living..

For transparency:
I had a dear friend my brother asked me to go into a triple role with him. While I am a pretty fresh professional, I have always had personal ethics that aligned with NOHS, so I stepped back without rushing and did what I was taught to do by my ethics instructors.
I consulted others, I did the fairness test and I did realize I would Absolutely do for every client what I was willing to do for him.
With that being said his story did have a very positive ending! Let me put my skills and lived experience to work for you!

This has been the best blessed feeling ever!
The honor I feel of being trusted to assist with the next transition of life for a dear friend, my brother Tony!!
Not sure one way or another what is going to happen or when, all we know is this is the final moment and it will be when God and him decide!!
Proud to be a certified Death Doula!! He has communicated that he is content, that he is the most comfortable he has ever been, he is not scared and thank you.

**PUBLIC SERVICE ANNOUNCEMENT**

If you have someone you love or a family member in the trusted care of an institution of any kind! Please go check up on them, check their safety, check to make sure they are o.k.

Unfortunately someone close to me has been mistreated and neglected by the very people we took him to for the help we needed to get insurance to cover safe placement. I am not sure where the break down happened and it really does not matter, communication is now happening somewhere else!
