Pathways offers comprehensive hospice care, palliative care, and grief and loss support for all, regardless of ability to pay. Throughout the past 40 years, we have built strong relationships with physicians, facilities, therapists, and other medical community members to provide the best care possible. We now serve thousands annually, including 1,200 hospice patients, 2,800 palliative care visits,

and 3,400 community counseling sessions. Caring for a loved one with an illness can take an emotional toll on a family. Pathways helps families find relief by providing a tenured team of trustworthy and loving caregivers across a wide range of services. As a non-profit organization, you can rest assured knowing we always put our patients above profits.

Longmont, we’re here.

For nearly 50 years, Pathways has been caring for Northern Colorado families through serious illness, hospice care, grief and some of life’s most tender moments. Now, we’re bringing that same compassionate care to Longmont.

With our expanded service area, individuals and families in Longmont can now turn to Pathways for hospice and palliative care, wherever they call home — whether that’s a private residence, assisted living or a skilled nursing community.

Our team provides personalized support that can include nursing and medical care, social work, spiritual care, grief support, volunteers and more, based on what matters most to each individual and family.

As we become part of the Longmont community, our local Community Liaison will be building relationships with hospitals, physicians, senior living communities and other organizations — while serving as a resource for patients and families who have questions about their care options.

Choosing hospice is personal, and quality matters. Pathways is proud to be a 4-Star Medicare-rated hospice based on family caregiver experience.

A great opportunity to connect, learn and share what we know about hospice care.

Pathways President Evan Hyatt recently attended a preview event for the Osher Lifelong Learning Institute (OLLI) at Colorado State University, where OLLI members had the opportunity to meet instructors and learn more about courses being offered this fall.

We’re excited to share that Evan has been selected to teach one of those courses: “Exploring Pathways Hospice, Palliative and Bereavement Care.”

On September 29, Evan will welcome OLLI members to the Pathways main office to explore hospice care, the hospice benefit and the support Pathways provides to patients and families.

Opportunities like this are a wonderful reflection of Pathways’ expertise and our commitment to being an active, engaged resource in our community. By sharing our knowledge and creating opportunities for conversation, we can help more people understand hospice, palliative and bereavement care before they need it.

Pathways is making an impact throughout Northern Colorado, and sometimes, the best evidence is simply how often our name comes up.

Earlier this week, Natalie Petersen, Pathways Director of Business Development, joined Brian and Tanner on Good Morning NOCO to talk about Pathways’ presence in the community and how frequently she hears friends, family and community members mention Pathways in their everyday conversations.

Natalie also shared more about A Vintage Affair, presented by Wilbur’s Total Beverage, our volunteer opportunities, and the upcoming Pathways Rediscovered thrift store.

It was a great conversation about the many ways our community connects with Pathways and supports our mission of providing compassionate hospice and palliative care.

It was a beautiful day filled with great food, drinks, activities, and plenty of opportunities to connect with physicians, healthcare professionals, families, and friends from throughout Northern Colorado.

We’re grateful to the Northern Colorado Medical Society for bringing the healthcare community together for this fun, family-friendly event — and we were happy to be part of it!

It was a great Saturday at the Johnstown YMCA! 💙

Pathways’ Shannon Neely, Patient Access Manager, and Natalie Petersen, Director of Business Development, spent the day at the Connected Senior Expo, joining more than 30 vendors and community organizations focused on connecting older adults, families, and community members with valuable resources.

From meeting with attendees to sharing information about Pathways and the support we provide throughout Northern Colorado, it was a wonderful opportunity to connect with the community we’re proud to serve.
