Pathways Hospice http://www.pathways-care.org Since 1978, Pathways has served the community with the utmost compassionate care.

Pathways offers comprehensive hospice care, palliative care, and grief and loss support for all, regardless of ability to pay. Throughout the past 40 years, we have built strong relationships with physicians, facilities, therapists, and other medical community members to provide the best care possible. We now serve thousands annually, including 1,200 hospice patients, 2,800 palliative care visits, and 3,400 community counseling sessions. Caring for a loved one with an illness can take an emotional toll on a family. Pathways helps families find relief by providing a tenured team of trustworthy and loving caregivers across a wide range of services. As a non-profit organization, you can rest assured knowing we always put our patients above profits.

For 25 years, Angie Sickmon has been part of the Pathways team, building a career rooted in compassion, teamwork and a deep commitment to the mission of hospice care.

From her early days as an RN Case Manager to leadership roles and now serving as a Clinical Documentation Specialist, Angie has supported patients, families and colleagues in many ways throughout her journey.

Strong partnerships help us provide the best possible care for patients and families.

Over the next several weeks, Pathways Hospice will be visiting UCHealth hospitals for resource fairs, connecting with clinical staff and other healthcare professionals who play an important role in coordinating care for patients in our community.

Last week, our wonderful nurses Jessica Moen and Yvonne Willemann represented Pathways at Poudre Valley Hospital in Fort Collins. These opportunities allow us to strengthen relationships, share information about our services, and continue working together to support patients and their loved ones when they need us most.

Last month, Pathways was grateful to host an event with the Sharing the Care Campaign of Northern Colorado, bringing our community together to learn more about dementia and Alzheimer’s disease.

We explored important topics including causes, treatments and the resources available to support individuals living with dementia and the families who care for them.

A heartfelt thank you to Peggy Budai and Danielle Hubbard for sharing your knowledge, insight and wonderful resources with us.

Your willingness to educate and support our community is truly appreciated.

By learning more about dementia and Alzheimer’s, we can better understand the experiences of those living with these conditions, support their families and approach each person’s journey with greater compassion and understanding.

Pathways Rediscovered is getting ready to open its doors in Loveland, and we need your help stocking the shelves!

Join us for a one-day Donation Drive on Saturday, November 14, from 10 a.m. to 1 p.m. at the Pathways Administrative Office, 305 Carpenter Road in Fort Collins.

We’re looking for gently used, reusable items including:

• Clothing and accessories
• Furniture
• Jewelry and watches
• Home goods
• Kitchenware
• Linens
• Seasonal décor
• Collectibles
• Books, DVDs, CDs and records
• Electronics
• Toys, games and sports equipment

Every donation will help bring Pathways Rediscovered to life while supporting compassionate care in our community.

Please note that some items cannot be accepted, including mattresses and beds, large appliances, computers and printers, cribs and car seats, medical devices, TVs and large exercise equipment, among others. See the full list in the attached flyer.

October is Breast Cancer Awareness Month. 💗

At Pathways, we work closely with oncology teams throughout Northern Colorado, supporting people and families through palliative care and, when the journey changes, hospice care.

Better outcomes matter throughout that journey. And when someone reaches the end of life, outcomes still matter: symptoms well managed, fewer crises, caregivers supported, wishes understood, and more meaningful time spent where and with whom a person chooses.

For our oncology partners, hospice can be an extension of the care and relationships they have built with their patients.

This month, we honor the whole story: those in treatment, survivors, those living with advanced disease, the people who love them, the professionals who care for them, and those we have lost. 💗

We were proud to be part of the Weld County Walk to End Alzheimer’s this past Saturday! 💜

Team members from Pathways joined community members, families, caregivers and supporters in coming together for an important cause: raising awareness and supporting those affected by Alzheimer’s disease and other forms of dementia.

Events like this remind us that caring for our community extends beyond the care we provide every day. We are grateful for the opportunity to stand alongside so many others who are working toward a future without Alzheimer’s.

Last weekend, Pathways team members joined friends, families and community members for the Larimer County Alzheimer’s Walk.

For Pathways, being part of events like this is about more than showing our support. It’s about standing alongside the families and caregivers in Northern Colorado who are navigating the realities of Alzheimer’s and dementia, and helping build a community where no one has to face that journey alone.

Alzheimer’s touches so many lives, and every step taken together helps raise awareness, strengthen connections and support the work being done to improve the lives of people living with the disease and the people who care for them.

What happens when community generosity, secondhand treasures and a mission of compassionate care come together? You get Pathways Rediscovered. 💚

Earlier this week, Natalie Petersen, Pathways Director of Business Development, and Kris Whitfield, Pathways Rediscovered Store Manager, joined Brian and Tanner on Good Morning NOCO to talk about the important role Pathways Rediscovered plays in our community.

More than a thrift store, Pathways Rediscovered gives new life to treasured items while helping support the compassionate care and services Pathways provides to patients and families throughout Northern Colorado.

Every donation, volunteer hour and purchase makes a difference.

Together, our community helps turn generosity into meaningful support for people and families when they need it most.
