If you’ve been trying to find your estrogen patch lately, you’re not alone. The current shortage is frustrating, and now we’re starting to see availability issues with some of the alternatives as more women are switching formulations.

A few things I recommend: call around to local pharmacies, check online pharmacies, and talk with your provider about other options like a different patch, gel, spray, systemic vaginal ring, or even a compounded option when appropriate.

Most importantly, don’t try to figure out the dose conversion yourself. These formulations aren’t necessarily interchangeable dose for dose. Talk with a clinician who is experienced in menopause care and can help you find a personalized option that works for you.

The goal is to plan ahead and find an alternative so you can stay on your estrogen without an unnecessary interruption.

One of my favorite parts of menopause care is watching the relief on a woman’s face when she realizes the answer is often much simpler than she expected.

These are three questions I answer almost every day:

• Can I use vaginal estrogen if I’m already on estrogen therapy?
• Do I need progesterone if I’m using vaginal estrogen?
• Why would I need vaginal estrogen if my hot flashes are gone?

Different hormones serve different purposes, and different tissues have different needs. The right treatment plan depends on your symptoms, your goals, your medical history, and your stage of menopause.

Education is one of the most powerful tools we have. When you understand why you’re taking a medication, you can feel more confident in the decisions you’re making for your health.

Vaginal estrogen is considered the standard of care for preventing recurrent UTIs in women with hypoestrogenism. Had some really meaningful conversations with patients this week.

The conversations this week kept coming back to perimenopause and menopause and how differently each woman experiences it.

That’s exactly why I don’t believe in a one-size-fits-all approach to hormone wellness.

The approach I take is layering. Starting with the basics usually one or two foundational medications and paying attention. How is your body responding? What side effects are showing up? What’s improving? Then we build from there, adding to the plan intentionally once we know how you’re tolerating what’s already in place.

It’s slower than handing someone a prescription and sending them on their way. But it’s so much more thoughtful. And the outcomes reflect that.

Perimenopause and menopause are not a problem to fix with a single solution. They’re a transition that deserves a provider who’s actually paying attention to you specifically.

That part of this work never gets old.
