Socially and medically complex patients present unique challenges to rural hospitals. Often, there are not enough resources or system navigation support to assist complex patients after discharge, and these patients are at high risk for readmission, and potentially poor outcomes. The Montrose Regional Health Care Transitions Program helps patients achieve success through advocacy, education and system navigation support. After one year, the program has seen remarkable success with a decreased readmission rate of 8.3%, patient/family satisfaction, and strengthened relationships with community providers and partners.
