Ingredients
1 medium Granny Smith apple, diced
½ cup dried cranberries
½ cup walnuts, toasted + roughly chopped
¼ cup crumbled goat cheese or feta
3 tbsp extra virgin olive oil
1 ½ tbsp balsamic vinegar

Fatty liver means excess fat has accumulated inside the liver cells. The newer medical term is metabolic dysfunction–associated steatotic liver disease (MASLD). It is often reversible, but the important question is whether there is also inflammation or scarring.

How does it happen?

The most common driver is insulin resistance, which causes the liver to receive and manufacture more fat than it can process. Risk factors include:

* Prediabetes or diabetes
* Abdominal weight gain
* Elevated triglycerides
* High blood pressure
* Sleep apnea
* Frequent sugary drinks, refined carbohydrates and ultra-processed foods
* Alcohol
* Certain medications
* Less commonly, hepatitis, thyroid disease or other liver disorders

Someone can develop fatty liver without obesity, and AST and ALT can be normal even when liver fat is present.

Lifestyle-medicine approach

* Follow a whole-food, Mediterranean or plant-forward diet: vegetables, beans, lentils, fruit, whole grains, nuts and seeds.
* Reduce soda, sweet tea, juice, added sugar, white bread, pastries and highly processed food.
* Replace saturated fats from processed meats, fried foods, butter and excess cheese with olive oil, nuts, seeds and avocado.
* Avoid or minimize alcohol while evaluating and reversing the condition.
* Exercise at least 150 minutes weekly, combining aerobic activity with resistance training two or three times weekly.
* If weight loss is appropriate, losing approximately 5% of body weight can reduce liver fat; 7–10% or more may improve inflammation and fibrosis.
* Address sleep apnea, diabetes, cholesterol, triglycerides and blood pressure.
* Unsweetened coffee may be liver-supportive if tolerated, but it is optional.

There is no proven “liver detox.” Be cautious with supplements because some can injure the liver.
