Our Community Health Worker, Farah, teamed up with Shante from New Horizon and Zhehao from Asian Community Care to visit local doctors’ offices and communities and connect with Medicaid members who may be eligible for housing support, transportation assistance, and nutrition services. 🏠🚗🥗
