🌿 Cannabis Use Disorder: Recovery Is Possible

Many people begin using cannabis to help with anxiety, insomnia, trauma, or stress. Over time, however, what once felt helpful can become something the brain begins to rely on.

Cannabis use disorder is a medical condition—not a lack of willpower.

Signs that cannabis may be becoming a problem include:
• Needing more cannabis to achieve the same effect (tolerance)
• Cravings or difficulty cutting back
• Continuing to use despite negative consequences
• Irritability, anxiety, or sleep problems when trying to stop
• Brain fog, decreased motivation, or poor concentration
• Using cannabis to cope with emotions rather than addressing the underlying issue

Treatment often includes:

💊 Can medications help?

At this time, there is no FDA-approved medication specifically for cannabis use disorder.

However, some medications and supplements may be considered on an individualized basis:

• Naltrexone has shown promise in some studies by reducing the rewarding effects of cannabis or decreasing cravings, although additional research is still needed.

• N-acetylcysteine (NAC) has been studied as a supplement that may reduce cannabis cravings, particularly in adolescents and young adults. While the evidence is mixed, some individuals may benefit as part of a comprehensive treatment plan.

❤️ ADHD, Cannabis & Stimulants: The Hidden Risks

One of the biggest misconceptions I hear is:

“Cannabis is natural, so it must be safe with my ADHD medication.”

The reality is that both stimulant medications (such as Adderall) and cannabis can affect the cardiovascular system—but in different ways.

🫀 Stimulant medications may:
• Increase heart rate
• Increase blood pressure
• Cause palpitations
• Increase the risk of arrhythmias in susceptible individuals

🟢 Cannabis (THC) can also:
• Increase heart rate (especially shortly after use)
• Cause changes in blood pressure
• Trigger palpitations or abnormal heart rhythms in some individuals
• Increase cardiovascular stress, particularly in those with underlying heart disease or cardiovascular risk factors

When these substances are used together, they may place additional stress on the heart. The risk can be even greater in people with high blood pressure, structural heart disease, arrhythmias, obesity, sleep apnea, ni****ne use, or a family history of cardiovascular disease.

Another important topic is dependence.

Both stimulant medications and cannabis have the potential to lead to dependence—but they do so differently.

Prescription stimulants can lead to tolerance or dependence when they are misused, taken in higher doses than prescribed, or used without appropriate medical supervision. Cannabis can also lead to cannabis use disorder, particularly with frequent or high-potency THC products.

The goal of this post is not to create fear or shame. It’s to encourage honest conversations between patients and their healthcare providers so treatment decisions are made with the best available evidence.

🧠 ADHD, Cannabis & Stimulants: The Brain’s Tug-of-War

One of the most common questions I hear is:

“If Adderall helps my ADHD, why doesn’t it seem to work when I’m using cannabis?”

Think of it like a game of tug-of-war happening inside the brain.

🟣 Stimulant medications (such as Adderall) work by increasing dopamine and norepinephrine activity in the prefrontal cortex—the part of the brain responsible for:
• Attention
• Focus
• Planning
• Organization
• Working memory
• Impulse control
• Executive functioning

The goal is to strengthen the brain’s “CEO,” allowing it to better organize thoughts, regulate behavior, and complete tasks.

🟢 Cannabis (THC) affects many of these same brain circuits—but often in the opposite direction.

THC can:
• Reduce attention and concentration
• Impair short-term memory and learning
• Slow processing speed
• Decrease motivation
• Alter executive functioning
• Affect judgment and decision-making

So while the stimulant is trying to improve these functions, THC may be making them more difficult.

⚖️ That’s the tug-of-war.

One substance is attempting to strengthen the brain’s executive control network, while the other may weaken the very same network. For some people, this means:
• The stimulant doesn’t seem to work as well.
• Higher doses may be requested even though the underlying issue is ongoing cannabis use.
• ADHD symptoms may appear to be “getting worse” despite appropriate medication.

This doesn’t mean every person who uses cannabis will have the same experience. The effects vary depending on factors such as dose, frequency of use, THC potency, age, and individual brain chemistry.

The goal isn’t to judge anyone—it’s to help patients understand how different substances may interact with the brain so they can make informed decisions about their treatment.

Sometimes the medication isn’t failing.

Sometimes the brain is being pulled in two different directions at the same time.

Many individuals with ADHD report using cannabis to help with anxiety, sleep, or to “slow their mind down.” While it may provide temporary relief for some people, research suggests that chronic cannabis use can also make ADHD symptoms more difficult to manage.

Cannabis may affect:

• Memory and learning
• Motivation and executive functioning
• Emotional regulation
• The effectiveness of stimulant medications in some individuals

A proper ADHD evaluation is not simply checking off symptoms on a questionnaire. A comprehensive assessment should include a thorough history and consideration of other conditions that can look similar to—or occur alongside—ADHD.

Before making an ADHD diagnosis, it’s important to evaluate for conditions such as:
• Anxiety disorders
• Depression
• Trauma (PTSD)
• Sleep disorders
• Autism spectrum disorder
• Bipolar disorder
• Medical conditions, learning differences, and other factors that may affect attention and behavior

An accurate diagnosis matters because treatment depends on understanding why symptoms are occurring. What appears to be ADHD may actually be another condition—or a combination of conditions—that requires a different treatment approach.

Borderline Personality Disorder (BPD) is one of the most misunderstood mental health conditions. 💜

Many people assume BPD is about “attention-seeking” or “manipulation,” but the reality is much more complex. Individuals living with BPD often experience intense emotions, fear of abandonment, unstable relationships, and a fragile sense of self. These symptoms are frequently rooted in early life experiences and are not a choice.

Some of the most commonly used supplements include:

🌙 Melatonin – May help regulate your sleep-wake cycle and can be beneficial for jet lag, shift work, or difficulty falling asleep.

💪 Magnesium (Glycinate or L-Threonate) – May promote relaxation, improve sleep quality, and support nervous system function.

🍵 L-Theanine – An amino acid that may help reduce stress and anxiety while promoting relaxation without causing daytime drowsiness.

🧬 Glycine – An amino acid that may improve sleep quality and help you wake up feeling more refreshed.

🌿 Valerian Root, Passionflower, Lavender, Chamomile, and 5-HTP have also been studied and may be helpful for some individuals, although the evidence varies depending on the supplement and the person using it.

Lithium has been used in psychiatry for decades and remains the gold standard treatment for Bipolar Disorder, particularly for acute mania and long-term mood stabilization.

One of lithium’s most important benefits is that it is one of the few psychiatric medications shown to reduce the risk of su***de in people with bipolar disorder.

🧠 Lithium may be used for:
✔️ Bipolar I Disorder
✔️ Acute manic episodes
✔️ Long-term mood stabilization
✔️ Treatment-resistant depression (in select patients)
✔️ Reducing suicidal thoughts and behaviors in appropriate patients

💊 Like all medications, lithium requires regular monitoring. Your healthcare provider will routinely check:
• Lithium blood levels
• Kidney function
• Thyroid function
• Electrolytes

⚠️ Staying well hydrated and maintaining a consistent salt intake are important while taking lithium. Dehydration, vomiting, diarrhea, certain medications (such as NSAIDs), and significant changes in sodium intake can increase lithium levels and potentially lead to toxicity.

Borderline Personality Disorder (BPD) is a complex mental health condition that affects emotional regulation, relationships, impulsivity, and self-image. While there is no FDA-approved medication specifically for BPD, medications may be used to help manage certain symptoms such as depression, anxiety, mood instability, impulsivity, and anger.

Depending on an individual’s symptoms, a provider may consider:
💊 SSRIs
🧠 Mood stabilizers
💜 Atypical antipsychotics
🩺 Naltrexone (in select cases)
🌿 PRN medications such as hydroxyzine
⚠️ Benzodiazepines are generally used cautiously and are often avoided because they can worsen impulsivity or increase the risk of dependence in some individuals.

One of the most important steps before starting treatment is making the correct diagnosis. Many psychiatric conditions can have overlapping symptoms, so it’s essential to complete a thorough evaluation before choosing a medication.

For example, before prescribing an SSRI, clinicians should evaluate for bipolar disorder or other mood disorders. In someone with bipolar disorder, an antidepressant such as an SSRI may trigger a manic or hypomanic episode, particularly if it is used without an appropriate mood stabilizer. That’s why a comprehensive psychiatric assessment is so important.

💜 Medication is only one part of treatment. Evidence shows that psychotherapy—especially Dialectical Behavior Therapy (DBT)—is a cornerstone of treatment for BPD, with medications used to target specific symptoms when appropriate.
