Hi everyone,

Quick update on our sports medicine services launch. We had hoped to open acute injury consultations by August 10th, but a few key supplies — splinting materials, casting materials, and equipment for ultrasound-guided injections — won't arrive in time to do this the right way. So we're pushing the launch to Friday, August 14th. I'd rather delay a few days than open before we're fully equipped to take care of you properly.

Once we open, we'll have two appointment options depending on your injury:

🏃 Sports Medicine Walk-In — Despite the name, this is by appointment only. Dedicated for fresh injuries within 72 hours, with 2 slots held each morning so you can get seen fast.

🩹 Acute Injury Consultation — Available throughout the day, for injuries within the past 2 weeks. Same great care, more flexible timing.

Both are similarly priced, and my consultation pricing is always bundled — your initial visit includes 6 weeks of direct text access to me plus follow-up, all built into one transparent price. No surprise add-ons.

LISM is cash-pay, so depending on your coverage, your current insurance might still be the cheaper option in some cases — but for patients with high-deductible plans, for example, LISM can actually come out ahead. And sometimes what insurance doesn't pay for is simply time. What I focus on is value, not a checklist.

"Wait, that's all it costs?"

That's the reaction I get almost every time I sit down with patients for a meet and greet.

They ask about lab costs, expecting hundreds of dollars — and I tell them it should cost significantly less at member rates. They ask about procedures, and I tell them most are covered under the membership. For specialized procedures, members get member rates — for example, an ultrasound-guided cortisone injection is $25. Compare that to the hundreds of dollars many patients have paid out of pocket for the same procedure elsewhere.

This is what happens when you remove the middlemen and build a direct relationship between doctor and patient. No surprise bills. No guessing games. Just transparent, honest care.

This is what Direct Primary Care is. I focus on value, not someone else's checklist.

Here’s something I’ve learned in years of practicing medicine: the nurse is the backbone of any clinic.

Think about it — when someone walks into a clinic, they’re usually not having their best day. They’re anxious, they’re in pain, they’re worried about what they’re going to hear. The nurse is the first face they see, the first voice they hear. That first interaction sets the tone for everything that follows. Get it right, and a patient feels like they’re finally in the right place. Get it wrong, and no amount of clinical expertise afterward fully makes up for it.

That’s why I’m so particular about who I bring onto the LISM team.

So I’m thrilled to officially announce that Pam Phillips is joining Lexington Integrative Sports Medicine as our nurse. I worked alongside Pam for six years, and I’ve seen her commitment to patients up close — the kind of care and work ethic you can’t teach. When she left her previous practice, patients noticed. That tells you everything about the kind of nurse she is.

And on top of that great news — we saw our second-ever DPC patient today. Small numbers now, but every patient is a step toward the vision I’ve had for what primary care should actually look like: unhurried, direct, and built around you.

For the last six years, I've practiced under the insurance-based primary care model. I've been able to help a lot of patients in that time — but a few things kept pulling at me, and they're a big part of why Lexington Integrative Sports Medicine is opening its doors on August 3rd.

Under the traditional model, an insurance company decides what I'm allowed to do for you. If you need more time, more attention, more of a workup — I can often provide it, but usually only after checking the boxes that were built by someone who has never met you. That checklist creates friction. For me, and for the patient.

Then there's the billing side. I've had patients tell me they were charged well into four figures for a simple injection, with no idea it was coming. Or they'll ask me upfront, "what's this going to cost?" — and honestly, all I can do is scratch my head, because in the traditional system, I often don't know either.

That's the gap I built LISM to fill. Direct Primary Care gives patients another option — one where you and I decide what your care looks like, not a checklist. No surprise bills. No middleman between us.

Did you know your HSA can now be used for Direct Primary Care? 💡

A recent federal tax law makes this easier than ever — though I always recommend checking with your tax advisor for your specific situation.

You can now pay your DPC membership with pre-tax HSA dollars. Simple and efficient.

On a traditional plan? Many employers offer FSAs, and those pre-tax dollars can also go toward DPC membership. Check with your employer if you're interested.

I stared at the screen for a solid ten seconds before it actually registered.

An appointment. Booked.

The first one on the calendar for Lexington Integrative Sports Medicine — before the doors have even opened.

It's not even day one yet — but somehow this moment feels bigger than opening day itself.

I think back to nearly six years in Georgetown, Kentucky — the athletes I've taped up on sidelines, the patients who let me be part of their recovery, the relationships that grew out of exam rooms and training rooms alike. I think about walking away from a full-time academic role that gave me stability, a title, a system that already knew how to run itself. Building something from scratch is a different kind of terrifying. There's no institution behind you. There's just the work, and whether people believe in it enough to show up.

So when I saw that first appointment land in the calendar, it hit differently than I expected.

Because I know exactly what it took to get here — and none of it was me alone.

It was some of the nurses I've worked beside for years, who looked at me and said, plainly, "You should do this. You'll succeed." That kind of confidence from people who've watched you work is worth more than they probably realize. It was friends who gave me their time and their skills for nothing in return except wanting to see this succeed — discounts on website work, honest feedback, small favors that added up to something big. And it was my family — my wife, who's absorbed more late nights and "just one more thing" conversations than anyone should have to; my brother and his family; my parents, who've supported this in ways big and small since long before "LISM" was even a name on a page.

None of that shows up on an appointment confirmation screen. But it's all in there.

I won't pretend I'm not nervous. Starting something from nothing means there's no safety net — just the belief that treating people like whole humans, not just a list of symptoms, is worth building a practice around. But nervous and excited have never been mutually exclusive, and right now I'm both, in equal measure.
