Join us for our upcoming webinar, Building California’s Trust Infrastructure: Reflecting on 10 Years of CACHI. As we mark 10 years of supporting Accountable Communities for Health (ACHs) across the state, we'll share what communities have learned about building the partnerships, infrastructure, and local capacity needed to advance health and equity.

We’ll hear from CACHI Executive Director Rea Pañares and longtime supporter Bonnie Midura of The California Endowment, followed by a panel conversation with ACH leaders:

AAPI Heritage Month is a time to honor the histories, leadership, and contributions of Asian American, Native Hawaiian, and Pacific Islander communities and to recognize the ongoing work needed to ensure that all communities can shape the systems that affect their health and well-being.

Across the ACH network, partners are working to build trusted, community-rooted infrastructure that reflects the diversity of California’s communities, including AAPI communities whose experiences are too often flattened, overlooked, or treated as invisible in health data and decision-making.

This month, CACHI celebrates the AAPI leaders, residents, organizers, providers, and community-based organizations advancing health equity, cultural and linguistic access, and community voice across the state. Their leadership reminds us that lasting systems change begins with communities being seen, heard, and resourced to lead.

We are proud to share CACHI’s inaugural impact report, 10 Years of Impact: 2016–2026, a reflection on what California communities have built together through the Accountable Communities for Health (ACH) model.

The report tells the story of what becomes possible when communities have the infrastructure, relationships, and shared leadership to move beyond isolated efforts and act together around a common vision for health and equity.

Since 2016, CACHI and our Accountable Communities for Health (ACH) partners have:

- Brought together 1,000+ cross-sector organizations statewide

- Engaged more than 20 Medi-Cal managed care plans

- Supported 20+ coordinated care systems, including CIEs, referral networks, and CHW/promotores models

- Helped communities respond to COVID-19, wildfires, and more together

- Shifted power to residents, placing community voice at the center of health decisions

Hear from ACH residents, community leaders, and partners, and learn about the 36 transformations ACHs have implemented across California—from securing $2M in USDA funding to expand green space access for low-income youth in Azusa, to training Healthy Housing Champions, in the Havenscourt neighborhood of Oakland, who won policy changes on lead abatement and rental inspection.

The stories featured in this report are just a few inspiring examples of how ACHs are creating lasting systems transformation by and for the people they serve.

Nos enorgullece presentar nuestro informe 10 Años de Impacto: 2016-2026, que documenta una década de trabajo para construir la infraestructura necesaria para promover la equidad en salud en California.

Desde 2016, CACHI y nuestros socios de las Comunidades Responsables por la Salud (ACH, por sus siglas en inglés) han:

- Reunido a más de 1,000 organizaciones de distintos sectores en todo el estado

- Involucrado a más de 20 planes de atención administrada de Medi-Cal

- Apoyado más de 20 sistemas de coordinación de la atención, incluidos CIEs, redes de referencia y modelos de CHW/promotores

- Ayudado a las comunidades a responder juntas al COVID-19, los incendios forestales y más
- Transferido poder a los residentes, colocando la voz de la comunidad en el centro de las decisiones sobre salud

This week, we're proud to spotlight the Antelope Valley Violence Prevention Coalition, a powerful example of community power and healing in action.

As an Accountable Community for Health, this ACH unites residents, providers, youth, and local institutions around violence prevention, healing, and safety, demonstrating what becomes possible when communities are trusted and supported to lead their own change.

Each tile in this mosaic was created by one of the 36 Accountable Communities for Health (ACHs) in CACHI's statewide network. We love how no two tiles look the same. ACHs are designed to be locally driven, shaped by the unique people, systems, visions, and challenges of each community. What they all share is a commitment to building the collaborative infrastructure and trust to advance change over time.

Hoy es el Día de los Trabajadores Agrícolas, un día para honrar a las personas cuyo arduo trabajo y manos alimentan a nuestras comunidades.

El movimiento de trabajadores agrícolas de California nació de la solidaridad intercultural: trabajadores filipino-estadounidenses liderados por Larry Itliong y Philip Vera Cruz se levantaron en Delano en 1965, junto a organizadores mexicano-estadounidenses como Dolores Huerta, para construir lo que se convertiría en los Trabajadores Agrícolas Unidos.

Juntos, lucharon y lograron protecciones que muchos de nosotros hoy damos por sentadas: salarios justos, condiciones laborales más seguras y el derecho a organizarse.

Hoy, los trabajadores agrícolas inmigrantes continúan siendo la columna vertebral de nuestro sistema alimentario. Sus contribuciones merecen nuestra gratitud, y sus comunidades merecen protección frente a la injusticia, no solo hoy, sino todos los días.

Today is Farmworkers Day, a day to honor the people whose hard work and hands feed our communities. The California farmworker movement was born from cross-cultural solidarity: Filipino-American workers led by Larry Itliong and Philip Vera Cruz stood up in Delano in 1965, joined by Mexican-American organizers like Dolores Huerta to build what became the United Farm Workers. Together, they fought for and won protections many of us now take for granted — fair wages, safer working conditions, and the right to organize. Today, immigrant farmworkers continue to form the backbone of our food system. Their contributions deserve our gratitude, and their communities deserve protection from injustice, not just today but every day.

When Jacqueline Franco-Silva first heard about East San José PEACE Partnership, it didn't have a name, a structure, or a plan. It was just an idea. What drew her in? Instead of being asked to react to something already built, she was invited to help build it.

From resident and community organizer to co-chair of the PEACE leadership table, and now Program Associate with CACHI supporting ACHs statewide, Jacqueline’s journey reflects what the ACH model is meant to make possible.

We recently sat down with her to hear:
- How promotoras became trusted COVID messengers in a county partnership that had never worked that way before
- What community-led governance actually looks like in practice
- How PEACE rapidly responded to COVID and today's immigration crisis
- What she's bringing to her new role supporting ACHs across California

Cuando Jacqueline Franco-Silva escuchó por primera vez sobre East San José PEACE Partnership, todavía no tenía nombre, estructura ni plan. Era solo una idea. ¿Qué fue lo que la atrajo? En lugar de que le pidieran reaccionar a algo ya construido, la invitaron a ayudar a construirlo.

De residente y organizadora comunitaria a copresidenta de la mesa de liderazgo de PEACE, y ahora Program Associate en CACHI, apoyando a las ACH en todo el estado, el recorrido de Jacqueline refleja lo que el modelo de Accountable Communities for Health (ACH) busca hacer posible.
