How do I know if my hormones are out of balance?

Sometimes you just know something is off.

You're not sleeping like you used to.
Your weight is changing.
You're more anxious or irritable.
You just don't feel like yourself.

So you get your hormones checked...

“Everything looks normal.”

But is that really the end of the story?

Not always.

This week I'm sharing a real patient example that shows why I don't just look at whether each hormone falls inside the “normal” range. How your hormones work together matters too.

The First Labs I Check When Someone Can't Lose Weight

When a patient tells me, "I'm doing everything right, but I still can't lose weight," there are two lab tests I almost always start with:

✔️ Fasting insulin

✔️ Fasting glucose

Most people are surprised to learn that fasting insulin isn't part of routine yearly blood work.

That means you can have a normal glucose, a normal A1C, and still have insulin resistance quietly making it harder to lose weight.

What's the first thing I look for when a woman can't lose weight?

It's probably not what you think.

Most people assume I start by checking thyroid hormones.

Or menopause.

Or cortisol.

Those are all important—but they're usually not the first place I look.

Just this week, I met with a woman who simply could not lose weight. She couldn't understand why nothing she was doing was working.

Her fasting glucose? 95 ✅

Her A1c? 5.4% ✅

Most providers would tell her everything looked normal.

But after looking a little deeper, we found significant insulin resistance.

Instead of immediately starting a weight loss medication, we focused on improving her insulin sensitivity first. We'll see how her body responds before deciding if medication is even necessary.

That's why I always say...

Normal doesn't always mean optimal.

One of my patients was struggling to lose weight despite doing many of the right things.

When we reviewed her labs, we found two important hormone imbalances:

✔️ Insulin resistance

✔️ Relative estrogen dominance

These weren't separate problems—they were connected.

Hormones work like an orchestra. When one section is out of sync, it affects the entire performance. In this patient's case, the imbalance between estrogen and progesterone likely contributed to her insulin resistance, making weight loss even more challenging.

Instead of treating just one issue, we addressed both by improving her insulin sensitivity and restoring a healthier balance between her hormones.

Have you spent money on "hormone balance" supplements but still don't feel like yourself?

One thing I see every week is women who have tried over-the-counter hormone products, oils, or supplements hoping they'll balance their hormones and finally relieve their symptoms.

The problem isn't necessarily that these products are "bad."

The problem is that they aren't personalized.

Before you can treat a hormone imbalance, you first have to identify which hormone is actually out of balance. Is it estrogen? Progesterone? Testosterone? Thyroid? Or is insulin the real issue?

Real Patient Case: It's Not Always About Just One Number

A male patient came to me after his primary care provider recommended starting a statin because his LDL cholesterol was elevated.

But when I reviewed his labs, I noticed something else.

His TSH was over 5 despite being treated for hypothyroidism.

Our hormones don't work in isolation. They're interconnected. When one hormone is out of balance, it can create a ripple effect throughout the body—including your cholesterol, energy, metabolism, and weight.
