Empty Cubital Tunnel? Where Did the Ulnar Nerve Go? 🤔

Have you ever scanned the cubital tunnel and couldn’t find the ulnar nerve behind the medial epicondyle?

Before assuming something is wrong, always ask about a history of ulnar nerve transposition. Surgical history completely changes where you should be looking. After transposition, the ulnar nerve is moved anterior to the medial epicondyle, and dynamic MSK ultrasound allows us to evaluate not only the nerve’s appearance but also how well it glides in its new location.

During your examination, look for:
✅ Smooth nerve gliding
✅ Stable anterior position without
compression or instability
✅ Preserved fascicular pattern

Be alert for:
🔴 Restricted movement
🔴 The nerve appearing stuck within
postoperative scar tissue, limiting its
normal movement
🔴 Kinking or recurrent compression
🔴 Enlargement or loss of the normal
fascicular pattern

One common answer is an enthesophyte—a chronic bony outgrowth that develops where a tendon or ligament attaches to bone.

Recognizing enthesophytes on ultrasound is an important skill because they can easily be mistaken for other abnormalities if you’re unfamiliar with their appearance.

⚽️ World Cup Injury Spotlight: Calf Injuries & MSK Ultrasound

Christian Pulisic’s recent calf injury is a great reminder of how common calf muscle injuries are in soccer—and how valuable musculoskeletal ultrasound can be in their evaluation.

When an athlete presents with acute calf pain, ultrasound can quickly identify the injured structure, detect fluid or hematoma, and most importantly, determine the grade of injury, helping guide treatment and return-to-play decisions.

One of the greatest advantages of MSK ultrasound is its ability to provide real-time, dynamic imaging while scanning directly over the patient’s point of maximal tenderness.

💡 If you’re learning MSK ultrasound, the calf is one of the best exams to master. Its superficial anatomy and predictable injury patterns make it an excellent place to build confidence in muscle imaging and tear grading.

In this reel, I walk through the ultrasound appearance of Grade 1, Grade 2, and Grade 3 calf strains and explain what to look for during your exam.
