Frozen shoulder, or adhesive capsulitis, occurs when the capsule surrounding the shoulder joint becomes inflamed, thickened and tight. This can lead to pain, stiffness and a progressive loss of movement, making even simple everyday activities increasingly difficult.

Sometimes, frozen shoulder develops without an obvious cause. In other cases, it may be associated with a previous injury or surgery, prolonged immobilisation, inflammation, diabetes, thyroid disorders or certain metabolic and neurological conditions.

It’s also more common in women, particularly around midlife and menopause, when hormonal, metabolic and inflammatory changes may influence how the body responds and recovers.

But what often gets overlooked is what happens beyond the shoulder.

When one part of the body stops moving well, the rest of the body adapts. Your neck, shoulder blade, spine, pelvis, hips and even your feet may begin to move differently as your body finds ways to compensate.

It is important to understand the benefits and risks of cold therapy for both men and women.

Cold therapy can trigger a powerful physiological response, with potential benefits for recovery, circulation, metabolism, inflammation and nervous system function.

But cold is also a stressor on the body and the intensity of that stress matters.

Your response can vary depending on temperature, duration, overall health, circulation, cardiovascular health, medications and individual physiology.

For women, hormonal fluctuations throughout the menstrual cycle may also influence thermoregulation and vascular responses.

The years leading up to menopause are an opportunity to build the physical foundation you’ll carry into the next stage of life.

From your mid-30s onwards, taking care of your muscle, bones, cardiovascular fitness, mobility and metabolic health becomes increasingly important. You may not feel the effects of hormonal changes yet, but the foundation you build now can influence how well your body adapts later.

After menopause, declining oestrogen can influence far more than the menstrual cycle. It can affect bone density, muscle mass, body composition, cardiovascular and metabolic health, brain function, energy levels, recovery and immune function.

Women are also disproportionately affected by autoimmune conditions, making this an important stage to be proactive about your health rather than waiting for changes to become a problem.

Fascia is the connective tissue that surrounds and connects the body. It contains sensory nerve endings that detect changes such as tension, pressure, stretch and movement.

This creates an important communication pathway between the body’s physical structures and the nervous system.

When you move, changes in the tissues create sensory input → that information travels through the nervous system → the brain and spinal cord process it → and the nervous system responds by adjusting movement and muscle activity.

Fascia therefore contributes to:
🧠 Sensory awareness: detecting mechanical changes within the body
🔄 Proprioception: contributing to our awareness of body position and movement
💪 Force transmission: helping distribute mechanical forces through connected tissues

- Challenge your muscles.
- ⁠Train your cardiovascular system.
- Move through your available range.
- Eat enough protein.
- Eat a diverse, nutrient-dense diet.
- Sleep and recover.

And keep asking your body to do things that require it to become more capable. Menopause changes your physiology, but it doesn’t take away your capacity to adapt.

That capacity is something worth protecting long before menopause and continuing long after it.

Pregnancy changes more than just your body shape, it can also change how your body moves, stabilises and distributes load.

Hormonal changes during pregnancy can increase ligament laxity, while the growing baby and shifting centre of gravity place additional demands on the body.

Together, these changes can affect how the pelvis, SI joints, hips and spine move and stabilise.

This may contribute to:
• Lower back pain
• Pelvic or SI joint discomfort

• Hip instability or discomfort
• Postural changes
• Movement compensations
• Increased susceptibility to injury

These changes are a normal part of pregnancy, but how your body adapts to them matters.

Movement patterns matter from the very beginning. Dysfunction and compensation can develop during childhood as movement patterns are learned and reinforced, influencing how forces are distributed and transferred through the body as we grow.

Uncorrected foot mechanics, such as flat feet, excessive pronation, or altered foot rotation, can contribute to problems later in life by changing lower-limb alignment and movement. These changes can influence how the knees, hips, pelvis, and spine absorb and transfer load, potentially contributing to pain, overuse and joint problems over time.

Common issues that may develop in adulthood include:

☑️ Plantar Fasciitis: Overpronation or flat feet can place increased strain on the plantar fascia, contributing to heel pain.

☑️ Patellofemoral Pain Syndrome: Altered foot mechanics can influence tibial and knee alignment, affecting how the kneecap tracks during movement and potentially contributing to pain.

☑️ Chronic Lower Back Pain: Altered foot mechanics can affect shock absorption and the alignment of the lower limb and pelvis, increasing mechanical demands on the lower back.

☑️ Bunions and Toe Deformities: Altered foot loading and pressure distribution can contribute to changes in the position and function of the big toe over time.

☑️ Posterior Tibial Tendon Dysfunction: Excessive pronation or a collapsing arch can increase the workload on the posterior tibial tendon, potentially leading to pain, weakness and reduced arch support.

We used to think of skeletal muscle primarily as a mechanical tissue that contracts, moves the skeleton, and burns energy. We now know that muscle is also an endocrine organ. When muscle contracts, it releases myokines, signaling molecules that communicate with tissues throughout the body.

One of the hormones produced by muscle is irisin. Irisin is being investigated for its role in glucose metabolism, insulin sensitivity, inflammation, mitochondrial function, brain health and the muscle–brain axis.

For women, this conversation becomes particularly important as they move through hormonal transitions such as menopause. Declining estrogen can alter body composition and metabolic health, making preservation of muscle mass and function increasingly important.

But here’s the nuance: Irisin isn’t the reason exercise works. Exercise creates a complex orchestra of signals, including myokines, metabolites, neural signals and hormonal responses. Irisin is one instrument in that orchestra.

We’re still learning exactly how important each instrument is. The bigger message is much simpler: Muscle is metabolic tissue. Muscle is endocrine tissue. Muscle is communication tissue. So don’t train only for appearance. Train for the messages your muscles send to the rest of your body.
