When your patient goes home with a PICC line or a port, the line care can come home too.

Our oncology and infusion nurses provide infusion therapy and PICC and port care in the home: flushes, dressing changes and site checks, on the schedule your plan of care sets.

Every visit starts with clean hands. CDC guidance on preventing catheter infections calls for hand hygiene before a line is accessed or its dressing is changed. Our nurses teach the patient and family the same habit, and the warning signs that mean a call.

October is National Physical Therapy Month. After a hospital stay for a heart condition, many people feel weaker than before. Getting stronger takes time, and it can start at home.

With your doctor's order, a home physical therapist can:

• build a plan around your own stairs, chairs and hallway;
• work on balance to help prevent falls;
• help you pace activity safely, as your cardiology team advises.

When you refer a patient to Oasis, our oncology nurses provide at home in Miami-Dade:

• infusion therapy and hydration;

• a nursing check on symptoms and medicines between oncology visits;

October is Talk About Your Medicines Month. If you see more than one doctor, such as an oncologist, a cardiologist and your primary care doctor, each one needs the full picture.

Keep one list and bring it to every visit:

1. Every prescription, with the dose and when you take it.
2. Over-the-counter medicines, vitamins and supplements.
3. Any medicine you stopped, and why.
4. Your pharmacy's name and phone number.

Before starting anything new, even a supplement, ask your doctor or pharmacist. At home, our nurses help you keep your list current. Serving Miami-Dade.

1. Which medicines changed, and why?
2. Which warning signs mean a call, and to whom?
3. When is the next appointment?
