Tonsillotomy may offer less pain and less bleeding than a traditional tonsillectomy. But if you're dealing with chronic infections or tonsil stones, leaving tissue behind may not always be the best answer.

The right operation depends on the right patient.

A patient recently shared something that I think many people with LPR can relate to.

After completing my reflux course, she experienced nearly a year of significant symptom improvement. Then, over time, some symptoms began to return.

Like many patients, her first thought was that a new supplement or medication might be responsible.

But after taking a closer look at her habits, she realized that some of her dietary triggers had slowly worked their way back into her routine.

This is one of the most important lessons I teach patients:

Reflux symptoms often return the same way they improve—gradually.

Most people are not derailed by a single meal. More often, it's the accumulation of small choices over weeks and months.

The good news is that once she recognized the pattern and returned to the habits that had helped her heal, she began noticing improvement again.

Success with LPR isn't about perfection. It's about consistency.

Can vitamin D affect your sinuses?

A recent systematic review and meta-analysis found that patients with chronic rhinosinusitis had significantly lower vitamin D levels compared to healthy controls. The study also found evidence that vitamin D supplementation may improve sinus symptoms in some patients over the short term.

Importantly, this does not prove that vitamin D deficiency causes sinusitis, nor does it suggest that vitamin D is a stand-alone treatment.

However, it highlights an important principle: chronic sinusitis is often influenced by multiple factors, including inflammation, immune function, allergies, anatomy, and possibly nutritional status.

Vitamin D may be another tool in the toolbox for some patients.

Today I returned to the campus where my journey toward becoming a physician began.

As I walked around, I found myself reflecting on how little I understood about the road ahead when I was a student here. At the time, I was focused on classes, exams, and the next immediate challenge. I couldn't possibly have seen all the opportunities, setbacks, relationships, and experiences that would follow.

It reminded me that we often want to know exactly where we're headed before we begin. Life rarely works that way.

More often, the path reveals itself one step at a time.

The older I get, the more I believe that the journey is not simply what gets us to the destination—it is the destination. The growth, the lessons, and the people we meet along the way are what ultimately matter most.

Sometimes it's good to return to where it all started and remember that.

Chronic ear pressure and plugged ears are often caused by Eustachian tube dysfunction.
A newer treatment option called balloon dilation aims to improve the function of the Eustachian tube itself. Clinical consensus statements from the American Academy of Otolaryngology have helped define which patients may benefit from this procedure.
As always, careful evaluation and patient selection are important.

A 2021 systematic review found no strong connection between H. pylori and LPR. Diagnosis should focus on symptoms and pepsin.

A 2021 study found salivary pepsin correlates with LPR symptoms and may help when pH probes aren’t accessible.

These simple “eat this, not that” swaps can help reduce pepsin activation and support long‑term LPR improvement.

Historically, all 2‑year‑olds stayed overnight after tonsillectomy. New research shows their complication rates match 3‑year‑olds — decisions should be individualized.

These are realistic LPR‑friendly meals that help reduce pepsin activation and support long‑term healing.
