Healed restoration tattoo after top surgery.

I’m proud to be a Gender Pathways preferred provider and to work with patients connected through Kaiser Permanente, OHSU and OutCare Health.

Kaiser Permanente and OHSU both have established gender-affirming care programs here in the Pacific Northwest, helping patients navigate care before, during and after surgery.

OutCare Health is a national LGBTQ+ health equity organization that connects patients with affirming, knowledgeable healthcare providers.

Cosmetic and reconstructive procedures come in a lot of forms, but the goal is often pretty simple: to feel more comfortable and confident in your own body. I’m happy tattooing can be part of that process.

We slapped some Bag Balm on it and went about our lives. You know… the stuff originally made for cow udders. 🐄💀

My client came to me with previous ar**la tattooing done elsewhere, and there was a lot we needed to correct.

The placement was too far to her sides. They were different sizes, sitting at different heights, heavily saturated and pulling orange. There was also no dimension to create the 3D effect.

The goal was to bring everything forward, create balance and symmetry, soften and shift the color, increase the size to better complement her body, and build the 3D detail that makes a restoration tattoo come to life.

Corrections like this require so much more than simply tattooing over what’s there. Every decision has to account for the existing placement, pigment and saturation while still creating something that looks intentional and belongs on her body.

This was a challenge.

And I am so proud of where we landed.

She can finally stand straight in front of the mirror and see what should have been there all along.

These will require another round of tattooing, to ensure we’ve balanced the color and hidden the old work. They’re going to look beautiful.

This is alot of information. But it’s important that clients and artists understand that scar camouflage is about so much more than matching skin tone.

Scar tissue has already gone through a major healing process. The collagen has been remodeled, the structure of the dermis has changed, and fibroblasts, blood supply, and immune activity can all behave differently than they do in healthy skin.

That matters when we tattoo it.

Pigment may hold beautifully in one area and heal lighter in another. It’s also why scar camouflage often takes more than one session. We need to see how the tissue heals before deciding what it needs next.

And when pigment heals lighter, it doesn’t necessarily mean the scar “pushed the ink out.” More often, we’re seeing differences in how remodeled tissue accepts and retains pigment.

Going deeper isn’t the solution either. More trauma can create more inflammation and increase the risk of texture changes, pigment migration, or additional scarring.

Scar camouflage should be thoughtful, conservative, and built in layers.

The goal isn’t to overpower the scar with ink. It’s to understand the biology well enough to seamlessly camouflage with it.
