Oh my goodness!!! These health insurance companies are something else! I am working on billing today for a client. We rec'd a denial because the patient needed a prior authorization and didn't have one. There are exceptions to this rule when it is medically necessary. I filed an appeal out of medical necessity. The insurance company denied my appeal saying the patient being admitted to an inpatient facility with a major cardiac event and not being able to verbally communicate was NOT a viable appeal reason and denied the claim AGAIN!

WHAT??? Oh no! That does not fly with me! I will pick this battle and fight it until we win!
