You came in to talk about hormones, and now your lab requisition has enough initials to win a game of Scrabble. CBC, CMP, TSH... why all of it?

I'm Sam, owner of Viking Alternative Medicine. Your provider needs a baseline before anything changes. When you change your testosterone, you're pulling a lever, and other levers move with it... your estradiol, your red blood cell count. And if one of your symptoms is feeling tired, low testosterone isn't the only possible reason. Your thyroid can do that too.

No single result decides your treatment plan. Your provider needs the whole picture, whether you're a man or a woman.

And before you order labs yourself, check with your clinic. Clinics get volume discounts from the testing facilities, and at my clinic we pass those savings on to you.

Forgot your requisition at the lab? Call your clinic and they can email you a copy. If you're with Viking, just give us a call at 210-826-8900 or visit vikingalternative.com.

Twice a day, six hours each time, on a normal workday. Put it on at 7 AM and you're off limits until 1 PM. Put it on again at 8 PM and you've got about two hours left, right around dinner. If the rule doesn't fit your life, talk to your provider about a delivery method that does.

Normal skin has collagen woven like a basket. Scar tissue is the emergency repair... a pile of ropes dumped into the hole, all facing the same direction. It's strong, but it's not the same as what was there before. That's one reason some product labels tell you to avoid injecting into scars.

If you use testosterone cream, it doesn't stay on you. It can move to anyone who touches the area, and to anything that touched it... a shirt, a towel, a pillowcase. We found out the hard way when my wife's labs came back over 1,500.

A hard spot where you inject is worth taking seriously. But the fix is an appointment with your provider, not a forum thread. If it's hard, red, hot or swollen, get it looked at.

One of the old wives' tales says to massage it out. In burn scars, studies have found massage can lead to a drop in pain and itching. But nothing concrete shows that massaging breaks up an injection lump.

Then there's vitamin E. We found six studies. Three showed the scar looked better, three showed no change, and in both cases a few people got a rash.

The bottom line... if a spot is hard, red, hot or swollen, make an appointment to see your provider. Don't diagnose it yourself.

So how did the rumor start that scar tissue blocks your testosterone? The answer is insulin.

People with diabetes who inject in the same spot over and over can end up with a fatty lump under the skin. A small study of 13 people showed insulin absorbed far less through those lumps than through normal skin.

It sounds like a logical jump to testosterone. But we're not talking about a fat lump, we're talking about scar tissue. And insulin and testosterone are very different substances.

You've been on TRT for a couple of years, and one night you feel it... a little hard knot under the skin, right where your favorite injection spot is. So you jump online, and somebody in a thread says that's scar tissue and it's blocking your testosterone.

That doesn't sound good. But is it true?

Scar tissue is real, and a hard spot is worth taking seriously. But as far as we can find, nobody has ever measured whether scar tissue at a testosterone injection site changes how much testosterone gets into your bloodstream.

The rumor started with insulin. People with diabetes who inject in the same spot over and over can end up with a fatty lump under the skin, and a small study of 13 people showed insulin absorbed far less through those lumps. It sounds like a logical jump to testosterone. But a fat lump isn't scar tissue, and insulin isn't testosterone.

If a spot is hard, red, hot or swollen, make an appointment to see your provider. Don't diagnose it yourself, and don't change your dose or your routine because of something you read online.

Six hours. That's how long the box said to wait before any contact with the area where the cream went on. And you apply it twice a day.

Put it on at 7 AM and you're off limits until 1 PM. Get home at 6, put it on again at 8, and you've got about a two-hour window. That window is competing with dinner.

Secondary exposure is real. It's the reason I don't use the cream anymore.

My wife is on hormone therapy too, and she started feeling a little off. On and off, for weeks.

Then her routine labs came back and her testosterone was over 1,500. That's where LabCorp stops counting. It's like they're saying your levels are so high, you probably don't want to know.

She wasn't using testosterone cream. I was. That's secondary exposure... someone picks it up from your skin, or from a shirt, a towel or a pillowcase you touched.
