Santa Clara Family Health Plan (SCFHP) is proud to celebrate four outstanding honorees for their dedication to our members’ care.

This year, SCFHP recognizes Andrea Cervenka, MD; Elizabeth McCraven, DO; and Farda Qureshi, MD; along with North East Medical Services (NEMS), a community-based nonprofit, for their excellence and commitment to serving county residents.

These honorees earned recognition based on their performance on SCFHP quality measures for calendar year 2025 and their strong results in SCFHP’s Provider Performance Program (PPP). Their work reflects a deep commitment to delivering high-quality, accessible care to our members.
