4 years ago, the day before my bday, I had cervical spine surgery. At the time, my doctors asked me if I had had trauma bc my MRI looked bad. I hadn't. My neurosurgeon told me after the surgery that he had done over 4000 cases and mine was the worst left sided herniation he had ever seen. I asked why. No one knew. So I started searching for my own answers. And that started what turned into a full time job of learning about Hypermobile Ehlers-Danlos Syndrome and all the related conditions!

OUCH!

Today I was just turning to clean my Pilates reformer machine after class, and I suddenly got a patellar subluxation. (Patella = kneecap. Subluxation = coming partially out of the joint). Unfortunately, this happens to me frequently (unrelated to Pilates or any particular exercise), and it’s painful! I got a classmate to pull my leg straight to reduce it.

Argh! It’s humbling to admit you haven’t been listening to your own advice. So here’s my post on patellar subluxation and now I have zero excuse not to work on this.

Why it happens?
It can happen because of differences in the way the kneecap (patella) sits in the groove (trochlea). But for us hypermobile people, it’s usually bc our ligaments are slacking on the job (ha ha - see what I did there? 😜). The patella may move out to the side of the body when the medial patellofemoral ligament is too lax.

How to prevent it?
It is very important to strengthen the quadriceps muscles, especially the inner group called the vastus medialis obliquus (VMO). Strengthening the gluteal complex — particularly the hip abductors and external rotators — also helps to ensure the knee moves in proper alignment.

Conversely, stretching of the IT (iliotibial) band, lateral quadriceps, hamstrings, and calf muscles can prevent the patella from being pulled to the side.
