Medication errors happen more often than people realize.
Yesterday while setting up medications for a client, I quickly noticed that 1 out of 3 bottles in a 90-day supply were the wrong dose.
Thankfully, this medication error was caught before it caused harm. But what if this had been a high-risk medication such as insulin, blood thinners, heart medication, or pain medication? Not everyone has a Veronica to set up their meds (although they should ☺️). The outcome could have been very different, especially for an older adult managing multiple prescriptions independently.
This is exactly why medication management matters. A quick “trust the label and take it” approach can be dangerous.

Pharmacies are staffed by humans, and humans can make mistakes. But even “small” medication errors should always be taken seriously because the wrong medication or wrong dose can quickly become life-threatening.
This situation ended safely because someone was paying close attention. Many seniors do not have that extra set of eyes.
