Good patient care starts with people who care about the patient, not just the paperwork.

At The Higher Care Clinic, we want your experience to feel personal, comfortable, and informative.

That means taking the time to answer questions.

It means explaining what comes next instead of assuming you already know.

It means creating an environment where patients can speak with a licensed medical professional and feel respected throughout the process.

And it means recognizing that every patient who walks through our doors has a different story.

Some people have participated in their state’s patient program for years. Others are walking into their very first appointment with absolutely no idea what to expect.

Both are welcome here.

Since opening in 2018, we’ve worked to create a clinic where education and patient support are just as important as getting through the appointment itself.

What actually happens during an appointment at The Higher Care Clinic?

If you’ve never had a physician evaluation before, it’s completely understandable to wonder what to expect.

Your appointment gives you an opportunity to speak with a licensed physician about your medical history and determine whether a physician recommendation is appropriate for you under your state’s program.

Our goal is to create an environment where you can ask questions, understand the process, and feel comfortable discussing your health without judgment.

And our support doesn’t necessarily end when your evaluation does.

Depending on what you need, our team can help explain your next steps, answer questions about required documentation, and help you better understand the state application process.

We’ve been doing this since Oklahoma’s program began in 2018, and we know that what feels routine to us may be completely new to you.

You don’t need to know everything before your appointment. That’s what we’re here for.

Over the last several posts, we’ve looked at inhaled, edible, sublingual, topical and transdermal formats.

They may involve some of the same compounds, but how those compounds enter the body can change how they are absorbed, metabolized and experienced.

An inhaled compound encounters the lungs.

An ingested compound travels through the digestive system and undergoes metabolism.

A sublingual formulation may be absorbed through tissues beneath the tongue.

A topical formulation is generally intended for localized application.

A transdermal formulation is designed to cross the skin barrier.

That’s why looking at a number on a product label alone doesn’t tell the entire story. Route of administration, formulation, individual metabolism and physiology can all influence onset, duration and individual experience.

Patient education isn’t about telling someone which format to choose. It’s about giving patients the information they need to understand the differences, ask informed questions and have better conversations with their healthcare professionals.

Both go on the skin, so what actually makes a product transdermal?

The difference is in what the formulation is designed to do.

Many topical products are intended primarily for localized application. Transdermal formulations, on the other hand, are specifically designed to facilitate movement of compounds across the skin barrier.

That isn’t an insignificant difference. The skin naturally functions as a protective barrier, so successfully moving a compound across it depends on factors such as the compound itself, product formulation and delivery system.

This is why the words topical and transdermal shouldn’t automatically be treated as synonyms.

Topical = applied to the skin for primarily localized use.
Transdermal = formulated to deliver compounds across the skin barrier.

The bigger lesson? Formulation matters just as much as where a product is applied.

When we say a product is topical, we’re describing where and how it is applied: directly to the skin.

Many topical formulations are designed primarily for localized application rather than delivery into systemic circulation.

The skin also happens to be a very effective barrier. Its outermost layer helps protect the body and limits the movement of many substances into deeper tissues and circulation.

That distinction becomes especially important when comparing topical and transdermal products. While both are applied to the skin, those terms don’t necessarily describe the same type of delivery.

In other words: simply being applied to the skin does not automatically make something transdermal.

“Sublingual” literally means under the tongue, but there’s a reason that placement matters.

The area beneath the tongue contains a rich network of blood vessels. Certain formulations are designed to remain in contact with these tissues so compounds can be absorbed through the oral mucosa.

This creates a different route than simply swallowing a product and relying primarily on digestion.

There is an important nuance, though: not everything placed under the tongue will necessarily remain there or be absorbed completely through the oral tissues. Some portion may ultimately be swallowed, meaning the formulation and the way a product is designed to be used can influence its absorption.

It’s another good example of why formulation + route of administration matter when comparing different modalities.
