We often encounter hoof imbalances, laminitis, founder, club feet, high-low hoof patterns, and many other hoof concerns. The question is: are these conditions the primary problem, or are they a symptom of something occurring elsewhere in the horse?

These anomalies may be human-induced, horse-induced, or a combination of both. Regardless of the cause, the hoof is rarely an isolated structure acting independently from the rest of the body.

This is why in Equine Visceral Osteopathy we study anatomy and physiology down to a cellular level. Understanding the body in depth allows us to identify not only the obvious symptoms we can see, but also the underlying restrictions and compensatory patterns that are not visible to the naked eye.

When evaluating a hoof, we look at the hoof as a whole, including all of its structures. Are they functioning together as intended, or is one structure being forced to work harder than the others? Is the hoof adapting to a restriction elsewhere in the body?

One of the most overlooked influences on hoof health is nutrition. The hoof is living tissue. Every cell that makes up the hoof is built from the nutrients the horse consumes daily.

The diet and what the horse is eating has a MASSIVE impact on the hoof. Time and time again, horses presenting with chronic hoof issues and sensativities are also presenting with dietary concerns. Whether it is excess sugars and starches, inflammatory ingredients, synthetic additives, toxins, poor-quality forage, or feeds that are simply not species-appropriate, the hoof almost always reflects what is occurring internally.

However, nutrition is only part of the equation.

A hoof can be "starved" of proper nutrition even when the horse is consuming an excellent diet. Nutrition must not only be consumed, it must also be transported, delivered, and utilized by the tissues.

Restrictions within the body can influence circulation, lymphatic drainage, neurological communication, and the movement of fluids throughout the horse. For example, restrictions involving the elbow, shoulder, thoracic inlet, fascia, or associated vascular structures may reduce the body's ability to efficiently deliver nutrients and oxygen to the distal limb while also affecting the removal of metabolic waste products. Do you know a horse with chronic abcesses?

As a result, a horse may be eating an appropriate diet, yet the hoof may still struggle to grow and function optimally because the tissues are not receiving everything they need. The question is not simply, "What is the horse eating?" The question is also, "Can the horse effectively deliver those nutrients to the tissues that need them most?"

The hoof is also directly influenced by the horse's overall anatomy and physiology. Restrictions within the stomatognathic system... including the teeth, TMJ, tongue, hyoid apparatus, and associated structures... the cranial system, fascia, visceral system, liver, and musculoskeletal system can all alter how forces are distributed throughout the body and ultimately expressed in the feet.

For example, a horse experiencing prolonged physical or emotional stress may develop increased tension throughout the stomatognathic system and fascial chains. Over time, this can influence posture, movement, circulation, and weight distribution. The horse may begin loading one side differently, shortening a stride, bracing through the shoulders, or shifting weight away from an area that no longer feels adaptable.

Months or years later, what the owner notices may be a high-low hoof pattern, persistent flaring, contracted heels, recurrent abscesses, uneven wear, or difficulty maintaining balance through routine trimming. The hoof becomes the visible expression of a compensation pattern that may have originated far from the foot itself.

Interestingly, we are often sought out only after traditional approaches have failed to produce lasting results.

By the time we see many of these horses, they may have undergone repeated trimming changes, corrective shoeing, medications, supplements, or other interventions directed solely at the hoof itself. While these approaches may temporarily improve symptoms, they often fail to address why the hoof adapted in the first place.

The hoof is rarely acting independently. It is responding to the horse's internal and external environment. If the underlying cause remains unchanged, the hoof will continue expressing the same compensation patterns regardless of how many times the foot itself is treated.

If direct trauma is involved, that adds another layer entirely. A physical injury creates immediate structural and neurological change. The horse will compensate around that trauma, often creating secondary restrictions in fascia, posture, and loading patterns. Even once the visible injury has healed, the compensation pattern may remain unless the entire system is addressed.

A hoof imbalance is rarely just a hoof problem. It is often part of a much larger story involving nutrition, circulation, stress, dental balance, visceral health, previous injuries, posture, movement, and the horse's ability to adapt.

The hoof tells a story. The challenge is learning how to read the entire horse, not just the chapter written in the foot.

When the cause is overlooked, the hoof becomes the focus. When the cause is identified, the hoof often begins to make sense.

Yet "rounding" teeth has become increasingly common in equine dentistry, often with the intention of removing sharp areas. The problem is that teeth do not develop sharp points and uneven wear patterns in isolation.

Nature designed the equine mouth very differently.

A Few Facts
The maxillary (upper) cheek teeth are naturally wider than the mandibular (lower) cheek teeth. Combined with the horse's characteristic lemniscate, or figure-8, chewing pattern, this creates a highly specialized system of wear that is unique to the species.

Unlike humans, horses do not chew straight up and down, nor do they chew on both sides of the mouth simultaneously. They chew one side at a time using a complex lateral grinding motion that is essential for proper function, digestion, and occlusal balance.

This chewing pattern, combined with the shape and biomechanics of the temporomandibular joints (TMJs), creates an angle across the cheek teeth known as the Angle of Wilson. Importantly, this angle is individual to each horse.

This is normal.

As a result, what many people consider "sharp" edges are often found on the buccal (cheek) side of the upper molars and the lingual (tongue) side of the lower molars. Their presence alone does not necessarily indicate pathology.

In dental school, I was taught to routinely remove these edges, even when they were not causing soft tissue trauma, ulceration, or discomfort. After years of studying osteopathy, I began to appreciate how much we often fail to consider when evaluating the mouth in isolation. Much much more should be taken into account.

I realized that removing tooth structure that was not causing a problem, creates new ones.

When we alter the natural contours of the teeth by rounding or flattening them, we are not simply changing the teeth. We are changing the biomechanics of the entire chewing apparatus.

The occlusal surface is the interface through which the horse interacts with its environment thousands of times every day. Changes to that surface alter mechanical input from the periodontal ligaments, muscles, joints, and surrounding tissues. The nervous system responds to this information continuously, adapting muscle tone and movement patterns accordingly.

When stimulation becomes uneven or altered, compensatory patterns may develop. Over time, this can contribute to changes in tissue tension, chewing mechanics, and occlusal balance.

Ideally, the teeth, periodontal structures, muscles, and joints should work together as a coordinated system, providing balanced sensory input and efficient function.

The TMJs are a particularly important part of this equation.

They are finely tuned joints that rely on proper dental contact and coordinated movement. When the shape of the occlusal surfaces is altered, the loading patterns on the TMJs change as well. These changes influence how forces are transmitted through the jaw, cranial bones, hyoid apparatus, musculature, fascia, and cervical spine.

Over time, these adaptations may contribute to dysfunction throughout the head and neck and may be associated with chronic tension patterns, performance limitations, and, in some horses, headshaking-type presentations.

From an osteopathic perspective, sharp points and uneven wear are often adaptations rather than primary problems.

Restrictions anywhere in the body, the poll, neck, thorax, limbs, pelvis, or even the visceral system, can alter posture and movement. These compensations ultimately affect how the horse loads the jaw and wears its teeth.

When we simply remove visible dental findings without addressing the underlying cause, we are often treating the consequence rather than the dysfunction that created it. In many cases, this does not solve the problem. It may even perpetuate imbalance by removing an adaptive wear pattern while leaving the original restriction untouched.

We ask a different question:

Why did this dental imbalance develop?

The answer is often not found in the mouth alone.

By identifying and addressing the primary restriction, wherever it exists in the body, we increase adaptability and improve the horse's ability to use itself efficiently.

Better movement creates better posture.

Better posture supports better jaw function.

Better jaw function supports healthier and more stable occlusion.

The goal should never be round teeth.

The goal is a horse that can use its body optimally and require less dentistry over time.

Healthy occlusion is not something that should be imposed. It is something that emerges when the entire system is functioning as nature intended.

One final observation: if your horse has unusually developed temporal muscles, it may be a clue that he or she has adopted a more vertical chewing pattern rather than the normal side-to-side grinding motion. The question then becomes not how to remove the resulting wear pattern, but why the horse stopped chewing the way nature intended in the first place.

Because when we stop focusing exclusively on teeth and start looking at function, we often discover that the mouth is telling us a story about the entire horse.

The teeth are part of a complex, integrated system that includes the TMJs, temporal bones, hyoid apparatus, cranial nerves, cervical spine, fascia, viscera, limbs, and hooves. Every day, we ask questions that extend far beyond the oral cavity.

How are the temporal bones positioned and moving?

How is the mandible relating to the cranium?

How is the horse moving through the occiput, atlas, axis, and upper cervical spine?

What patterns are being expressed through the poll?

Is the hyoid apparatus compensating or decompensating?

How are the cranial nerves functioning within their surrounding tissues?

Could restrictions within the cranial or cervical systems be influencing the trigeminal system and the horse's comfort, mastication, or performance?

Does the body demonstrate compensatory or decompensatory patterns?

Are the thorax, diaphragm, viscera, pelvis, and limbs contributing to the horse's presentation?

Are the hooves trimmed and balanced in a way that complements the individual horse, or is the body being asked to compensate?

How is the horse organizing itself against gravity?

The poll is one of the most neurologically and biomechanically significant regions in the horse. It serves as a meeting point between the cranium and body, where information is constantly exchanged through the nervous system, fascial system, musculoskeletal system, and sensory apparatus. As a result, it often reflects the adaptive strategies being expressed throughout the entire horse.

From an osteopathic perspective, dentistry is not simply about identifying sharp enamel points (thats the easy part), it is about understanding the relationships that exist between structure and function throughout the body. How to improve mobility throughout to increase function and adaptability for the individual horse.

No two horses are the same.

The mouth may be where we begin our assessment.

But to truly understand the horse, we must look beyond the teeth.

Because the question is not whether the teeth are connected to the rest of the body.

The question is: how could they not be?

The carotid arteries carry blood from the heart to the brain. If blood flow is restrictedโ€”because of misalignment, compression, or other issuesโ€”the brain may not get enough oxygen and nutrients.

Key Arteries:

โ€ขExternal carotid artery (ECA): feeds the face, jaw, and other head structures

Hidden within the core of the skull, the sphenoid sits in relationship with nearly every cranial structure... articulating with 12 cranial bones... helping coordinate balance, symmetry, jaw function, and the subtle motion of the head as an integrated system.

Within its structure are many openings called foramina (small passageways that allow critical nerves and blood vessels to enter and exit). Among them is the pathway associated with the optic nerve, directly linking to vision itself. These are not incidental details; they are reminders of how densely interconnected this region is.

When a structure like the sphenoid moves, rotates, or is restricted, it is reasonable to appreciate that even subtle shifts in its relationships could influence the function of nearby neural and vascular pathways within the cranium.

And when you begin to appreciate thatโ€ฆ something shifts.

Because the horse does not separate the skull from the body.
Or the jaw from the spine.
Or the teeth from the way the whole system organizes itself.

It is all communication.

The sphenoid becomes a key point in that conversation... its position and relationships influencing cranial alignment, TMJ balance, tongue and hyoid function, cervical posture, and the deeper patterns of tension and adaptation that travel beyond the head.

On its inferior surface sits a small, saddle-shaped depression called the sella turcica (Latin for โ€œTurkish saddleโ€). A remarkably fitting image in equine anatomy. This structure houses the pituitary gland, a central regulator of endocrine signaling.

The pituitary acts as a messenger interface between the brain and the rest of the body, helping regulate hormone production across multiple systems. In particular, the anterior pituitary releases adrenocorticotropic hormone (ACTH), which signals the adrenal glands to produce cortisol... a key hormone involved in metabolism, stress response, growth, and body composition.

This is where the โ€œlightbulb momentโ€ happens.

When we realize that what presents as a localized issue (such as dental imbalance or asymmetry) may in fact be part of a wider network of cranial relationships, adaptive processes, and regulatory function.

Suddenly, the horse is no longer seen in pieces.
But as one continuous, intelligent, compensating whole.

And dentistry becomes something more than maintenance of enamel.

It becomes an opportunity to observe function.
To recognize compensation.
To understand how structure and system are constantly negotiating balance.

When the sphenoid is understood in this way, it changes how we look at the horse entirely.

Because sometimes the most important structures are not the ones you see firstโ€ฆ

They are the ones everything else is listening to.
