Even though all acne is 'hormonal' (it results from imbalances in your androgen hormones that stimulate excess sebum production), we usually refer to 'hormonal acne' as the increased breakouts you experience around your period.⁠

This kind of acne results from the normal decrease in oestrogen around menstruation. So there's less oestrogen available to suppress normal androgen levels, and androgen-stimulated sebum production increases.⁠

So if you have any microcomedones (invisible blocked pores), this extra sebum can be enough to block things up completely. And extra sebum is the equivalent of a 10 course tasting menu for acne bacteria to feed on. This kicks off the cascade of inflammation that leads to visible pimples (if you want to learn more about how this works, check out our Acne Treatment Guide on the website).⁠

What's the best way to treat hormonal breakouts?⁠

Chat to your doctor about methods to reduce hormonally-driven sebum production like the oral contraceptive pill, spironolactone or isotretinoin/Roaccutane. If these aren't for you, then your best bet is preventing microcomedone formation with regular, and long-term retinoid use to clear out gunky pores and keep them clear (check out our earlier post on why retinoids don't work as a spot treatment though).⁠

Even if you're using a topical retinoid, you can sometimes still get a few period spots. If you're prone to hormonal breakouts, using a benzoyl peroxide cleanser on your breakout-prone areas (usually the chin/jawline) the week before and during your period can keep your acne bacteria population in check for added anti-pimple insurance!

Anyone with melasma knows that religiously using sunscreen doesn't stop pigmentation relapse (even with 👒👕🏖️🕶️)⁠

In the last post we showed that specific wavelengths and dosages of light darken pigment and cause it to persist, especially in darker skin tones. These bad boys are UV (esp. UVA1) and visible light (VL, esp. blue light) from the ☀️⁠

Do sunscreens with VL + UVA1 filters prevent melasma relapse?⁠

One study showed that subjects using a broad spectrum VL/UVA1 sunscreen + a 4% hydroquinone fading cream had better pigment improvement than with a broad-spectrum untinted sunscreen + hydroquinone alone.⁠

Another trial compared a broad-spectrum chemical UVA1 sunscreen, with a tinted version of the same sunscreen. Those using the tinted sunscreen had less melasma relapse after 6 months.⁠

Several lab bench studies showed that tinted physical sunscreens filter VL better than non-tinted physical sunscreens, especially the nano/transparent ones.⁠

Other studies showed similar results but, because VL testing isn't standardised, and artificial light sources were used to mimic ☀️, it's difficult to be absolutely certain of these results.⁠

I have melasma, what's the best sunscreen?⁠

This is new research, but there's enough early scientific consensus to suggest melasma sufferers should use sunscreen with UVA1/VL filters:⁠

1️⃣ UVA1: multiple filters block this wavelength (320-340nm). There's a great chart linked in bio so you can check the filters in your fave sunscreen.⁠

2️⃣ Blue light: Untinted, zinc and titanium dioxide sunscreens alone don't protect against VL unless the particles are large enough to turn your skin white. So if you are using an untinted, transparent physical sunscreen, it's not doing the best job. Likewise untinted chemical sunscreens. Iron oxide tints are the only known VL-filtering ingredient available, especially in >3% concentration.⁠

So if you have melasma, look for a broad-spectrum SPF50+ tinted sunscreen that also includes specific UVA1 filters.⁠

Melasma is considered a chronic skin condition because without ongoing management it will return (and you'll waste 💸 on fading treatment). Melasma pigment is also exquisitely sensitive to light in all forms.

🛑Warning, there's hectic science ahead. Here's the summary: if you have melasma, especially with a darker skin tone, you'll benefit from a sunscreen that specifically blocks UVA1 and blue light from the ☀️. (More about this tomorrow)

What's blue light?

Let's start with radiation types at the earth’s surface:

1️⃣ UV Radiation (UVR) comprising UVA + UVB (UVC is filtered by the atmosphere so doesn't reach us). UVA can be further divided into 2 wavelengths: UVA1 (340-400nm) and UVA2 (320-340nm). Broad-spectrum sunscreens focus on filtering wavelengths

Am I too young to start a prescription retinoid?⁠

Have I left it too late?⁠

These are some of the most common questions we are asked. And there are loads of opinions about the 'right' age to add a prescription retinoid to your routine. But, like skincare specifically designed for 'mature' skin, retinoids don't know your age!⁠

When considering a retinoid, stop worrying about whether you've hit the right age (or passed it!). Instead, focus on your skin health, condition and goals.⁠

There are a variety of prescription retinoids (not just ), and dosages, to treat:⁠
✔️Blackheads, congestion, acne or hormonal breakouts⁠
✔️Early signs of ageing, texture, uneven skin tone, (this can be visible by your early 20s [1])⁠
✔️Advanced sun damage (deeper lines, wrinkles, sagging, rough skin skin, sun spots)⁠
✔️Pigmentation⁠
✔️Dull, lacklustre skin⁠
✔️Enlarged pores⁠

There's no upper time limit for using them either! Prescription retinoids work best to *prevent* skin damage, so you can start using a retinoid at a young age, and keep using it forever. In fact, the best results happen after 6-12 months of regular use. Retinoids are safe to use for decades (and no, you can't run out of skin cells, in case you've heard that old myth 🙄) [2]⁠
