Part 1: How Our Days Start

1. A few snoozed alarms
2. A quiet drive (maybe a bit rushed because of, well, 1 👆🏻).
3. The first (maybe second) coffee
4. Watching the morning light spill over Sandton
5. Excited to see our patients ...

... Patients: “Do I really have to stop playing padel after my PRP?” 👀

(coming up next week - things we get asked daily).

"I slept funny"

Have you ever woken up and suddenly can’t turn your head? That’s often what we call an acute wry neck. We also often blame our sleep.

In many cases, it is driven by irritation of the small joints in the neck (facet joints), combined with a protective muscle response.

The facet joints guide movement between each level of the cervical spine. When one becomes irritated, the surrounding muscles tighten to protect it (and movement suddenly feels restricted and painful).

It’s usually not one single movement or your sleep that causes it. More often, it’s a build-up of load (posture, screen time, stress) with a small movement acting as the trigger.

The neck is highly mobile and very sensitive to change: which is why even a small joint irritation can feel quite dramatic.

In most cases:
• Gentle, pain-free movement helps
• Avoid prolonged static positions
• Gradually restore normal motion

If symptoms persist or worsen, seek assessment from a qualified healthcare practitioner, such as a biokineticist.

James’ Journey

As a child, James developed osteomyelitis: a bone infection that affected the structure and growth of his leg.

Now, to address the long-term structural effects of the condition, and to keep him playing his beloved game of golf, James had internal correction surgery, six months in an external fixation frame, and 3 months with us...

Throughout that time, movement and strength work remained an important part of the process. Training continued with the frame in place to maintain mobility, muscle strength, and joint function while the bone healed.

The fixation has now been removed, and the next phase of rebuilding strength and function continues.

Recovery takes patience, consistency, and progressive work over time. Above all: a posting attitude, just like James.

Rotator cuff pain isn’t just “inflammation.”

Badge-type pain + a painful arc (60–120°) often indicate load intolerance in the cuff tendons.

Important distinction:
• Tendinitis = reactive / inflammatory
• Tendinopathy = reduced load tolerance + structural change

Tendons don’t strengthen with rest. They adapt to mechanical load. Thus, our rehab focus:
• Heavy, pain-controlled isometrics
• Slow, progressive eccentrics
• Restore force balance + correct asymmetry

Assessment guides load. Load drives adaptation. Tendon repair takes time and structured progression.

Unsure where to start? Seek assessment from a qualified healthcare practitioner, such as a biokineticist.

NO MORE.

No more looking away.
No more staying quiet.
No more normalising what should never be normal.

We stand in quiet solidarity.
For those who have been silenced.
For those still finding their voice.
For the change that’s long overdue.
