Right now, 6 of our mentees are running active practices with structured Diabetes Care & Management and Digital Health fully in place seeing patients, generating income, doing the work they trained for.

And 4 are already onboarded with Medical Aids for Patient Support Programmes and Chronic Care Management which means recurring, sustainable income built into their practice model.

This is what mentorship is supposed to do: turn a qualification into a career you actually own.

Why are we still struggling to manage metabolic disease effectively?

It is easy to say that patients are “non-compliant” or that diabetes and obesity are simply becoming more prevalent.

But the problem is much bigger than the patient.

The failure to prevent and manage cardiometabolic disease often sits at the intersection of several gaps:

🔹 Awareness gaps – Patients may not understand their risk until complications develop.

🔹 Health literacy gaps – Knowing a diagnosis is not the same as knowing what to do with it every day.

🔹 Diabetes education gaps – Education is often treated as an add-on rather than an essential part of clinical care.

🔹 Healthcare workforce capacity – We need more healthcare professionals who are confident and competent in chronic disease prevention and management.

🔹 Skills gaps – Screening, lifestyle counselling, medication education, behaviour change support, technology and clinical decision-making require practical skills.

🔹 Fragmented care – Patients often move between healthcare professionals without a coordinated approach to their long-term care.

And this is where nurses have an enormous opportunity.

The future of chronic disease care will require nurses who can do more than execute clinical tasks.

We need nurses who can educate, coach, assess risk, interpret data, use technology, coordinate care and lead prevention strategies.

If we want better patient outcomes, we must also invest in building the capability of the healthcare professionals delivering that care.

The question isn't only: “How do we get patients to change?”

It is also:

“Are we equipping our healthcare professionals with the knowledge, skills and confidence to help them change?”

That is the conversation we need to have.

Wrapped up Tonight's Live Mentorship class in our Digital Campus . We explored in depth on Glucose Monitoring- Glucometer and Continuous Glucose Devices. The importance of Remote monitoring also plays a part of in depth support in Diabetes Management to improve education and coaching strategies for the people living with diabetes.
