The hand/forearm with severe burns or skin loss has been sutured into the abdominal wall. The goal is to use the well-vascularized abdominal skin as a donor site to cover the wound and allow new blood vessels to grow into the damaged hand before separating it later.

😭After 2-3 weeks when the flap has its own blood supply, surgeons do a second surgery to divide the pedicle and reconstruct the hand.

📚 Administration of Magnesium sulfate ⬇️ ⬇️ ⬇️ ⬇️

According to Pritchard regimen for the prevention of preeclampsia and management of eclampsia

1. LOADING DOSE 14 g total
♦️ IV Dose 4g out of this 14g will be given IV.

💉 The magnesium sulphate comes in %

Using 50% magnesium sulphate: 4 g = 8 mL.

You are going to add 12 mL of sterile water for injection (Aqua) to make 20 mL.

Give slowly IV, usually over about 5 minutes.

Do NOT add lignocaine to the IV dose.

♦️ IM dose the remaining 10 g

Using 50% magnesium sulphate: 10 g = 20 mL.

Give 5 g (10 mL) deep IM into each buttock.
The IM dose is given with lignocaine to reduce pain.

Right buttock: 10 mL MgSO₄ + 1 mL of 2% lignocaine

Left buttock: 10 mL MgSO₄ + 1 mL of 2% lignocaine

2. MAINTENANCE DOSE— 5 g IM every 4 hours

Using 50% MgSO₄, 5 g = 10 mL.
Give 10 mL deep IM in one buttock.

Add 1 mL of 2% lignocaine to reduce pain.

Alternate the buttocks with each subsequent dose.

No Aqua is needed for the IM maintenance dose.

For those giving Diclofenac and Arthemeter injection IV ( intravenously) this is what happened to the hands.

Pls be careful Diclofenac and Arthemeter injection are for IM only ( intramuscularly) not IV. This is an act of negligence nd unprofessional attitude, pls don’t play with any body’s life 💔🥹.
