It’s a groundbreaking, unique case report of a surgical technique I developed over 20 years for complex congenital hip dislocation. A remarkable case of a neglected femoral neck fracture, resulting in a 6.7 cm leg length discrepancy.

For years, the patient lived with severe shortening and functional limitation.
Due to the high risk of nerve injury, surgical intervention was considered too complex.

Using a refined surgical approach adapted from techniques applied in congenital hip dislocation a total hip replacement was performed with controlled limb length restoration.

The result:

12 years later — back on the operating table 🦴

Imagine living 12 years with a hip replacement, and then – the pain comes back. That’s exactly what happened to my patient. The acetabular cup had gradually loosened, destroying the bone around it. Aseptic loosening — one of the most challenging scenarios in revision surgery.

When I opened the joint, the picture was tough: the acetabulum was severely damaged, barely any bone stock left. Yet the femoral stem? Still solid as a rock 💪
This is where the real surgical challenge begins. I used 2 fresh-frozen femoral heads for compact bone grafting — essentially rebuilding what had been lost over the years. Only then was I able to fit a primary cementless cup with excellent primary stability.

Revision surgery isn’t just about “swapping a part.” It’s about restoring what seemed permanently lost 🔬​​​​​​​​​​​​​​​​

Joint replacement is a procedure which involves replacing a damaged articular surface with an artificial implant. This surgery will restore the knee alignment and the range of motion. It is very effective for pain relief and patients can progressively go back to normal life activities.
🫵This surgery needs long expertise in the field and hyperspecialized team. The literature demonstrated that high volume surgeons have very low rate of complications compared to surgeons performing less than 50 surgeries a year.
