All of this information about us is kept in various databases by a variety of providers, pharmacies, insurance companies, local, state and national governments. We don't have access to this data, but third parties do, often without our awareness and we carry the brunt of the risk and responsibility for its cleanup if it's lost or exploited.

Patient misidentification is a problem that is seldom understood by people, especially from a non-healthcare perspective.

Health systems run into several cases where thousands of patients share the same name and birthdate. That was the case for "Maria Garcia" at Harris Health System in the Houston area. The system has records for 2,833 Maria Garcias—and 528 of them have the same birth date. The system has 138,000 cases where two or more patients share the same birth date and first and last name.

And this is just one instance, patient misidentification and lack of medical history contribute to 136 million patient harm events globally each year.
