دنوکانو دفنګسی امراضو تداوی : اکثره دفنګس ضد کریمونه او ملهمونه مفید نه تمامیږی، نو باید په سبستمیک ډول دخولی له لاری دفنګس ضد درملو نه استفاده وشی.
مثلا Terbinafine او itraconazole دفنګس ضد درملونه په عام ډول سره زیات استعمال لری او موثر واقع کیږی نسبت نورو درملو ته.

What’s the difference between folate and folic acid?

Folate is a general term for a group of water soluble b-vitamins, and is also known as B9. Folic acid refers to the oxidized synthetic compound used in dietary supplements and food fortification, whereas folate refers to the various tetrahydrofolate derivatives naturally found in food. (1)

The form of folate that can enter the main folate metabolic cycle is tetrahydrofolate (THF). (2) Unlike natural folates, which are metabolized to THF in the mucosa of the small intestine, folic acid undergoes initial reduction and methylation in the liver, where conversion to the THF form requires dihydrofolate reductase. The low activity of this enzyme in the human liver, combined with a high intake of folic acid, may result in unnatural levels of unmetabolized folic acid entering the systemic circulation.

Several studies have reported the presence of unmetabolized folic acid in the blood following the consumption of folic acid supplements or fortified foods. (3) Human exposure to folic acid was non-existent until its chemical synthesis in 1943, and was introduced as a mandatory food fortification in 1998. (4) Food fortification was deemed mandatory due to overwhelming evidence for the protective effect of folic acid supplementation before conception and during early pregnancy on the development of neural tube defects (NTD) in newborns.
Risks associated with excessive folic acid intake

While the incidence of NTDs in the United States been significantly reduced since folic acid fortification began, there has been concern about the safety of chronic intake of high levels of folic acid from fortified foods, beverages and dietary supplements. (5) One of the major risks associated with excessive intake of folic acid is the development of cancer. (6) In patients with ischemic heart disease in Norway, where there is no folic acid fortification of foods, treatment with folic acid plus vitamin B12 was associated with increased cancer outcomes and all-cause mortality. In the United States, Canada, and Chile, the institution of a folic acid supplementation program was associated with an increased prevalence of colon cancer. (7, 8) A randomized control trial found that that daily supplementation with 1 mg of folic acid was associated with an increased risk of prostate cancer. (9)

Researchers have hypothesized that the excessive consumption of folic acid in fortified foods may be directly related to the increase in cancer rates. Excess folic acid may stimulate the growth of established neoplasms, which can eventually lead to cancer. The presence of unmetabolized folic acid in the blood is associated with decreased natural killer cytotoxicity. (10) Since natural killer cells play a role in tumor cell destruction, this would suggest another way in which excess folic acid might promote existing premalignant and malignant lesions.

A high intake of folic acid might mask detection of vitamin B12 deficiency and lead to a deterioration of central nervous system function in the elderly. In one study, consumption of folic acid in excess of 400 micrograms per day among older adults resulted in significantly faster rate of cognitive decline than supplement nonusers. (11) Another study found a higher prevalence of both anemia and cognitive impairment in association with high folic acid intake in older adults with a low vitamin B12 status. (12) As vitamin B12 deficiency is a common problem for many older adults, these studies suggest that high folic acid intake could cause serious cognitive consequences in the elderly.
Folate from natural food sources is best

Despite the risks associated with high levels of folic acid intake, it is well established that adequate folate intake from the consumption of folate-rich foods is essential for health. Folate aids the complete development of red blood cells, reduces levels of homocysteine in the blood, and supports nervous system function. It is well known for its role in preventing neural tube defects in newborns, so women of childbearing age must be sure to have an adequate intake prior to and during pregnancy.

Excellent sources of dietary folate include vegetables such as romaine lettuce, spinach, asparagus, turnip greens, mustard greens, parsley, collard greens, broccoli, cauliflower, beets, and lentils. (13) Not surprisingly, some of the best food sources of folate are calf’s liver and chicken liver.

You can supplement with folate if your dietary intake is inadequate. Look for products that contain the Metfolin brand, or list “5-methyltetrahydrofolate” or “5-MTHF” on the label. Avoid products that say “folic acid” on the label. Make sure to check your multivitamin, because most multis contain folic acid and not folate.

Women planning on becoming pregnant should consume between 800 and 1200 mcg of folate per day for several months before the start of pregnancy. Unless you’re consuming chicken or calf’s liver and substantial amounts of leafy greens on a regular basis, it’s difficult to obtain this amount from diet alone. If you’re pregnant or trying to get pregnant, I recommend supplementing with 600-800 mcg of folate per day, depending on your dietary intake. Solgar is a good brand, but there are several others that typically use 5-MTHF including Designs for Health, Thorne, Metabolic Maintenance and Pure Encapsulations.

All other people, such as men and older women, should be able to get plenty of folate in a diet with adequate vegetable consumption, and do not need to supplement.

The immune system is the body’s way of protecting itself from infection and disease. Smoking compromises the immune system, making smokers more likely to have respiratory infections.
Smoking also causes several autoimmune diseases, including Crohn’s disease and rheumatoid arthritis. It may also play a role in periodic flare-ups of signs and symptoms of autoimmune diseases. Smoking doubles your risk of developing rheumatoid arthritis.
Smoking has recently been linked to type 2 diabetes, also known as adult-onset diabetes. Smokers are 30% to 40% more likely to develop type 2 diabetes than nonsmokers. Additionally, the more ci******es an individual smokes, the higher the risk for diabetes.

When a man can't get an er****on to have s*x or can't keep an er****on long enough to finish having s*x, it's called erectile dysfunction. Erectile dysfunction is the preferred term rather than the more commonly used term of impotence. Sexual dysfunction is often associated with disorders such as diabetes, hypertension, coronary artery disease, neurologic disorders, and depression. In some patients, s*xual dysfunction may be the presenting symptom of such disorders. Additionally, ED is often an adverse effect of many medications.
Although there are a number of options available for non-surgical treatment of s*xual dysfunction, it is clear that oral therapy has revolutionized the treatment approach to men with erectile dysfunction.
The FDA approved Vi**ra in 1998, and it became the first oral therapy for erectile dysfunction on the market. Vardenafil hydrochloride (Levitra) was approved in September, 2003, and Tadalafil (Cialis) was approved in November, 2003. Vi**ra, Levitra and Cialis work in much the same way. These PDE-5 inhibitors share many similarities, but they have differences as well.
Although sildenafil citrate (Vi**ra), vardenafil HCl (Levitra) and tadalafil (Cialis) sometimes are called s*xual enhancers, they do not directly cause an er****on of the p***s, but alter the body's response to s*xual stimulation by enhancing the effect of the nitric oxide, a chemical that is normally released during stimulation. Nitric oxide causes relaxation of the muscles in the p***s, which allows for better blood flow to the pe**le area.
PDE-5 inhibitors can be used to treat s*xual dysfunction caused by high blood pressure, problems affecting nerve function, prostate surgery, side effects of medicines, emotional or psychological ED. Although they work in the similar way, they are not the same in all respects. Sildenafil and vardenafil have similar molecular structures, but tadalafil is structurally different.
Vi**ra (Sildenafil)
• The success rate of sildenafil amounts to an average of over 80%.
• Several studies have shown a benefit of Vi**ra (sildenafil) for women with s*xual arousal disorder, including an increased ability to lubricate, achieve or**sm, and experience s*xual satisfaction.
• Onset of action: 30-60 minutes.
• Duration of action: up to 4 h.
• Fatty food is known to delay the absorption of Vi**ra.
• Contraindicated in persons taking organic nitrates; alpha-blockers (in the current labeling the interaction is considered a Precaution.)
• Vision disturbances are more common after taking Vi**ra, than after other PDE-5 inhibitors. In July 2005, the FDA found that sildenafil could lead to vision impairment in rare cases and a number of studies have linked sildenafil use with nonarteritic anterior ischemic optic neuropathy.
• The patent on Vi**ra expires in the early 2012.
Levitra (Vardenafil)
• Vardenafil is more potent and selective biochemically than other PDE-5 inhibitors.
• Onset of action: 25 minutes (the quickest).
• Duration of action is up to 5-6 hours, little longer than that reported for Vi**ra.
• Not recommended in patients taking type 1A (such as quinidine, or procainamide) or type 3 antiarrhythmics (such as sotalol, or amiodarone).
• The advantage that vardenafil has over sildenafil is that it does not alter color perception, a rare side effect which occurs with sildenafil (because vardenafil does not inhibit phosphodiesterase-6).
• Vardenafil is contraindicated in patients taking organic nitrates, alpha-blockers (in the current labeling the interaction is considered a Precaution).
• Fatty food delays the absorption of vardenafil. However, it is less sensitive than Vi**ra to food intake.
• The patent on Levitra expires in 2018.
Cialis (Tadalafil)
• The most astounding feature of tadalafil is its length of action. It has long half life, which ensures 36-hour duration of effectiveness, allowing for more flexibility and spontaneity in s*xual relations. This feature greatly distinguishes Cialis from its two predecessors Vi**ra and Levitra.
• Onset of action: 16 to 30 minutes.
• Tadalafil has the lowest prevalence of visual side effects. However, the myalgia seems to be more common with tadalafil, approaching 10%.
• Tadalafil is contraindicated in patients taking organic nitrates, alpha-blockers.
• Cialis is protected by a patent until 2016.
From a safety perspective, all three Vi**ra, Levitra and Cialis share similar drug interactions involving CY3A4 inhibitors and a contraindication in the use of nitrates. The side effects are similar for all three PDE-5 inhibitors and include headache, flushing, nasal congestion, and dyspepsia.
Which is better Vi**ra, Cialis or Levitra?
Cialis clearly works for 36 hours or more, making it an ideal choice for men who are very s*xually active.
For men who develop angina and therefore might require an organic nitrate (such as nitroglycerine), the longer duration of action is a disadvantage. Also, some people are uncomfortable having the active ingredient in their system for an extended period of time.
Levitra works a little longer than Vi**ra, and gives a quite sufficient window of effectiveness for spontaneous s*xual experience. Vardenafil is the most potent molecule of all PDE-5 inhibitors and works better in hard-to-treat cases of impotence [2].
Vi**ra clearly works for women with s*xual arousal disorder [1].
Implicit advantage of Vi**ra: Many men have been ordering high-dose pills and splitting them, because Pfizer's Vi**ra blue pills are pretty large (14mm) and easy to split.
Levitra, a small hard orange 8mm pill, is much more difficult to cut. Cialis 12mm pills are also inconvenient to split because of their asymmetrical almond shape.
The Key
Keep in mind that you don't have to make a definitive choice and stay on one ED medication all the time. You can choose all three and alternate between them depending on your requirements. For example, Vi**ra or Levitra on weekdays and Cialis on weekend or holiday.
