Preparing for an NDIS plan review or choosing a new provider? This practical checklist may help.

Before your review, consider:
☐ What supports are working well?
☐ What has changed in your daily life, health, goals or circumstances?
☐ What barriers are making it harder to participate, work, study or live independently?
☐ What evidence, reports or progress updates could support your requests?
☐ Are your current budgets being used effectively, and are any supports unavailable or unsuitable?

When speaking with a provider, ask:
☐ How will you tailor support to the participant’s goals, culture, communication style and preferences?
☐ What happens if a support worker is unavailable?
☐ How are concerns, incidents and feedback handled?
☐ Are your workers appropriately trained for the required level of support?
☐ What are the costs, cancellation terms and service agreements?
☐ Will the participant remain in control of decisions about their support?
