1st Wednesday September Q & A

Here at the Watson Headache® Institute, we appreciate everyone who joined us today from the US, Chile, New Zealand and Australia for our regular Q&A sessions discussing all things headache, migraine and their comorbid conditions.

This morning, topics ranged from ‘Red Flags’, onset of new headache over 50 years of age and the worst migraine ever and, for the first time, prolonged hemiplegic and speech aura in an established history of migraine in a 75-year-old patient, to idiopathic eye syndrome and the enigmatic tinnitus condition.

Thank you for your contribution; you are what makes these sessions so valuable, and so is the increasing number joining us.

Time to Chat

We all gathered socially after the second day of the course which we love to do.

This gave us time to chat and hear more stories about professional and personal lives.

Wonderful to hear that course attendees visited Glenelg, some staying to watch the sun set in the west, something that can't be done when living on the east cost of Australia.

Thank you to everyone who could join us before going to dinner with each other, family and or friends.

C0–C3: Why the upper cervical spine is the clinical entry point in headache and migraine assessment.

The atlanto-occipital (C0–C1), atlanto-axial (C1–C2), C2–C3 zygapophyseal joints and sub occipital muscles (particularly obliquus capitis inferior) are richly innervated by upper cervical afferents that converge with trigeminal afferents at the spinal trigeminal nucleus, the Trigeminocervical Complex (TCC).

Sustained nociceptive input from dysfunctional upper cervical structures points toward a mechanism for afferent barrage into the TCC, potentially driving and maintaining central sensitisation in cervicogenic headache, migraine, and related primary headache disorders.

Clinically, this convergence anatomy proposes that:

Segmental reproduction of familiar head pain at C0–C3 points toward cervical afferent involvement

Skilled manual assessment and treatment of C0–C3 may reduce afferent load and modulate the sensitised brainstem state over time.

The brainstem cannot be manually assessed or treated.

The upper cervical spine can.

This is the clinical logic at the foundation of the Watson Headache® Approach.

Neurophysiological Evidence and the Placebo Question

The placebo question is one of the most persistent challenges in manual therapy research, and it's one Dr. Dean Watson's PHD (Musculoskeletal Physiotherapist) own data has directly addressed.

In his 2014 study with Professor Peter Drummond, the nociceptive blink reflex, a brainstem measure patients cannot consciously influence, showed clear, significant changes in trigeminocervical excitability following upper cervical manual techniques.

Independent replication from Jafari and colleagues has since reinforced the same finding using similar neurophysiological markers.
