Central Sensitisation ~ Common Features

WIDESPREAD PAIN
Pain felt in areas beyond the original site of injury.

HEIGHTENED SENSITIVITY
Greater response to normally non-painful stimuli (e.g. touch, pressure, movement).

PERSISTENT SYMPTOMS
Pain that continues even after the original cause has healed.

ASSOCIATED FACTORS
Often linked with sleep disturbance, stress, mood changes and fatigue.

COMMON CONDITIONS
Seen in conditions such as chronic migraine, fibromyalgia, chronic low back pain, and persistent post-injury pain.

1st Wednesday October Q & A

The Missing Postdrome: A Clue to Cervical Relevance in 'Migraine'

Another great Q&A session, this time centred on a patient whose response to treatment illustrates a point I think is often overlooked. Following manual cervical intervention, she went on to experience a migrainous episode, but, notably, without the postdrome that had consistently followed her attacks in the past. The usual hangover of fatigue, cognitive fog and lingering discomfort simply didn't eventuate.

This matters. The absence of a previously reliable postdrome suggests that the cervical input had altered the underlying process, not merely the headache itself, and points to the relevance of the cervical spine in what has been labelled 'migraine'. What followed supports this: she has now been episode-free for over four weeks, where previously a week was the most she could manage.

Even more telling, exposure to lavender, until now a guaranteed trigger, failed to provoke an attack. Taken together, these changes reflect a meaningful shift in her sensitised state and suggest we are influencing the brainstem mechanisms that drive the condition, rather than simply managing its symptoms.

What Does Anatomy Warn Us Not to Miss?

0C3 will demonstrate a great deal to you should you examine it honestly; such things as reproduction, resolution, and whether the pain moves in the same way as cervical pain does.

But it will never show you what the history had refused to reveal. In the case of a first headache appearing at an age when, especially if it is migrainous in nature, such headaches are supposed to be rare, and when the proposed explanation is a little too simple; if you’re in any doubt at all, then investigate.

That is not caution merely for the sake of caution, it is our responsibility.
