The first sign may not be a symptom.

Health changes don’t always begin with something obvious.

Sometimes, the first indication that something is different is a change in a person’s usual presentation.

It could be:

• A change in blood pressure
• Unexpected weight loss or gain
• Reduced appetite
• Changes in blood glucose, where clinically relevant
• New or worsening pain
• Reduced mobility or increased falls
• Needing more assistance than usual

But health monitoring goes beyond clinical measurements.

Changes in sleep, hydration, mood, behaviour, energy, communication or daily functioning can also provide important information.

That’s why knowing a person’s baseline matters.

When you know what is normal for someone, you are better placed to recognise when something has changed.

The goal isn’t simply to collect numbers. It’s to notice patterns, document changes clearly and escalate concerns appropriately when needed.

Know the baseline.
Notice the change.
Look at the whole person.

A diagnosis doesn’t always tell the whole story.

A Functional Capacity Assessment (FCA) looks at how disability affects a person’s everyday life not just their diagnosis.

It may look at six key areas:

• Communication
• Learning
• Mobility
• Self-care
• Self-management
• Social interaction

It can also explore things like personal care, decision making, work and community participation.

An FCA can help provide evidence about a person’s abilities, challenges and support needs.

And importantly, not everyone in the NDIS needs an FCA.

Because understanding a person’s support needs means looking at how they function in everyday life not just what their diagnosis is.

The plan changes before the day has properly started, a small achievement feels like a huge win, and somehow you’ve found five different ways to explain the same thing.

But underneath the funny moments are some pretty important ones too.

You learn patience.
You learn to adapt.
You learn that trust takes time.
And you learn that sometimes the best support is simply being present.

No two days are exactly the same and neither are the people we support.

What is evidence, and why does it matter in the NDIS?

When people hear the word “evidence,” they often think only of medical reports.

But useful evidence is really about helping the NDIS understand a participant’s everyday experience.

It can help explain how disability affects daily functioning, what assistance is currently required, what has changed, why a particular support may be needed, and what outcomes that support is intended to achieve.

Evidence may come from healthcare professionals, therapists, assessments, progress reports, or other relevant information depending on the participant’s circumstances.

More paperwork doesn’t always mean better evidence.

What matters is that the information is clear, relevant, current and connected to the participant’s circumstances and support needs.

Independence looks different for everyone.

We often think of independence as being able to do everything without help.

But independence can mean completing part of a daily routine, making a personal choice, learning a new skill, communicating a preference, participating in the community, or deciding how support should be provided.

For some people, the right assistance is what makes greater independence possible.

Quality disability support should create opportunities for participation, build confidence, respect individual choices and support the person to remain in control.

Progress should never be measured by comparing one person with another.

Every person has different strengths, goals and support needs.

Choosing an NDIS provider is an important decision and it’s okay to ask questions before you commit.

The right provider should understand your goals, respect your choices, communicate clearly, and make you feel supported.

Before choosing a provider, ask the questions that help you understand:
✔️ What support they offer
✔️ Whether they understand your needs
✔️ How they tailor support to your goals
✔️ How they communicate with participants

Because the right support can make a real difference.

When does a change in mobility need clinical review?

Changes in walking, standing, transferring, strength, or balance should not automatically be attributed to tiredness, disability, ageing, or a bad day.

The important question is whether the person’s mobility is different from their usual baseline.

A participant may suddenly require more assistance, appear unsteady, avoid movement because of pain, or struggle with an activity they could previously complete.

Support workers, carers, and families can help by observing and documenting:

• When the change began
• What the person can no longer do
• Any pain, weakness, dizziness, or numbness
• Whether a fall has occurred
• Recent illness or medication changes
• Whether the change is worsening

Falls can cause serious injury, and severe falls may require emergency care.

Sudden weakness, particularly on one side, difficulty speaking, collapse, or sudden loss of balance may indicate a medical emergency. Call Triple Zero (000) immediately.

An NDIS plan reassessment considers whether a participant’s current plan continues to reflect their disability-related needs, goals and circumstances.

The process may involve reviewing progress towards goals, changes in daily functioning, current support arrangements and relevant evidence from health professionals or service providers.

Clear, current and specific information can help demonstrate what has changed, what is working and where additional support may be required.

Following the reassessment, the participant’s plan may remain unchanged, be varied or be replaced with a new plan.

Understanding the process and preparing supporting information early can help participants approach reassessment discussions with greater confidence.

Medication safety is about more than remembering to take your medicines.

Safe medication management involves understanding why a medication has been prescribed, recognising potential adverse effects, maintaining an accurate medication record, and ensuring medicines continue to meet your current health needs.

Medication-related harm remains a significant patient safety concern, yet many risks can be reduced through education, regular medication reviews, and open communication with healthcare professionals.

Five simple reminders:

✔ Know why each medication has been prescribed.

✔ Be aware of potential side effects.

✔ Keep an up-to-date medication list.

✔ Have your medications reviewed regularly.

✔ Ask questions before making any changes.

If you experience unexpected symptoms or have concerns about your medications, speak with your GP or pharmacist before stopping or changing your treatment.

Life changes,and sometimes your NDIS supports need to change too.

Many participants don’t realise that changes in their health, goals, or personal circumstances can affect the supports they need.

Keeping your support coordinator informed allows your support team to respond early, coordinate services effectively, and ensure your supports continue to align with your goals.

Some examples include:
✔️ Changes to your health.
✔️ New or different goals.
✔️ Increased support needs.
✔️ Moving house or changes to your living arrangements.
✔️ Concerns about your current supports.
