๐Ÿ’œ Meet Juliann Passmore โ€“ Behaviour Support Practitioner

Support should start with understanding the person, not simply focusing on behaviour.

Weโ€™re pleased to introduce Juliann Passmore, a Behaviour Support Practitioner at Transform Life.

Juliann supports teenagers and young adults, with experience across trauma-informed practice, complex behaviours, supported independent living, self-harm support, LGBTQIA+ support, risk assessment, crisis response and de-escalation.

She also has experience supporting Aboriginal and Torres Strait Islander communities and people, with a strong focus on respectful, culturally responsive and person-centred support.

Juliannโ€™s approach is person-centred, trauma-informed and strengths-based. She takes the time to build trusting relationships and understand each individualโ€™s experiences, needs, strengths and goals.

Working collaboratively with participants, families and support networks, Juliann focuses on building confidence, independence and meaningful participation in everyday life.

Weโ€™re excited to have Juliann as part of the Transform Life team. ๐Ÿ’œ

โœจ A wonderful Christmas in July celebration with our Launceston team!

Our Launceston team came together for a special evening of good food, great company and plenty of festive cheer.

A huge thank you to Briana Stewart for putting so much effort into making the night special for everyone. It was a lovely way to wrap up our three weeks of Christmas in July team dinners and celebrate the wonderful people who make up Transform Life.

A big thank you to Kylie Riseley, Nikki Jeffrey, Carleigh Burns, Isobel James, Tia McGillvery, Doreen Makombe, Briana Stewart, Liam Clark, Steph Searle, Bailee Atkinson-Fox, Kayla Carpenter and Jemma Curwen for being part of the celebration.

When Claire first began receiving support, engaging with others felt overwhelming. Over time, consistent and individualised support has helped her build trust, confidence and meaningful connections.

Claireโ€™s journey is a reminder that progress doesnโ€™t always happen all at once. Sometimes, it starts with feeling safe, being understood and having the right support around you. ๐ŸŒฑ

A lovely evening with our Adelaide and Launceston teams!

Thereโ€™s something special about taking time away from the day-to-day to connect, share a few laughs and celebrate the people behind the work we do.

It was wonderful to see Chelsea Talbot, Yolanda Januarie, Erin Haas and Gayathri Rajesh come together for a relaxed evening and enjoy some quality time as a team.

A meaningful therapy goal starts with one important question: What matters most to this person?

Therapy goals can support much more than clinical progress. They can help a person build confidence, communicate more effectively, return to meaningful activities, develop independence and participate more fully in everyday life.

The SMART framework โ€” Specific, Measurable, Achievable, Relevant and Time-Bound โ€” can help create clear and achievable goals. But the most important part is making sure those goals are meaningful to the individual.

At Transform Life, person-centred allied health support considers each personโ€™s strengths, needs, priorities and everyday experiences.

Because progress isnโ€™t about ticking boxes. Itโ€™s about creating meaningful outcomes that make a difference in real life.

It was so lovely to see our Transform Life Sydney team come together for an evening filled with great company, plenty of laughs and genuine connection.

Team connection is such an important part of creating a positive workplace culture, and weโ€™re grateful for the opportunity to spend time together outside of our day-to-day work.
